TIME DELAY RELAYS In accordance with 10 CFR 21.21(b)(2), the Sacramento Municipal Utility District (SMUD) hereby submits a written report documenting the notification made by Mr. C. Stephenson of the District to Mr. L. Miller of Region V Nuclear Regulatory Commission (NRC) on September 26, 1986 concerning the deficiencies found in the Syracuse Electronics time delay relays installed in the Transamerica Delaval Inc.

(TDI) Standby Diesel Generators. The diesel generators are not yet in service at Rancho Seco.

During pre-operational startup testing, an internal inspection of two (2) erratic Syracuse Electronic time delay relays, model TER-03803NL, detected several deficiencies. Relays TD-1B and TD-2B (reference SMUD' drawing M17.02.1-511, sheet 6) could not be calibrated. A flat spot on the side of the calibration potentiometers' mounting holes, intended to restrain rotation, had worn away allowing the potentiometers to rotate and short their contacts. A relay was also found with no solder on one point of its internal circuit board. One of the non-class IE time delay relays had an incorrect coil voltage rating shown on its name plate (110VDC versus 115VDC specified) and exhibited incorrect operation (indicating LED on its cover lit prior to the end of the timing cycle).

A nonconforming report (NCR), S-5020, was written to address the deficiencies.

The NCR was assigned to the Nuclear Engineering Department for resolution. As part of Nuclear Engineering's resolution, TDI was requested to investigate whether a 10 CFR 2i notification is warranted. Additionally, Nuclear Engineering initiated an internal occurrence description report (0DR),85-474, requesting a 10 CFR 21 reportability assessment be made by SMUD personnel.

The ODR was reviewed on September 3, 1986, by the Plant Review Committee (PRC).

The PRC recommended the deficiencies identified for relays TD-1B and TD-2B in the nonconforming report be considered a defect in a basic component and recommended a report be provided to the NRC pursuant to 10 CFR 21.

The defect identified by the PRC was a deviation in a basic component (i.e.,

diesel time delay relays) in that improper calibration of the time delay relays may have rendered the TDI diesel engine inoperable due to premature activation of the shutdown circuitry.

The repairs included placing a lock washer.on the calibration potentiometers to prevent rotation. In addition, a work request has been generated to inspect the remaining time delay relays of this kind in the two (2) generator control

Attached is a list of nuclear plants supplied with the Syracuse Electronic time delay relays.
