In accordance with 10 CFR 21.21(b)(2), the Sacramento Municipal Utility District hereby submits a written report to document a potential problem with Gems Sensors Model No. 63328A Modular Receivers.

Gems Sensors is stocking a nonqualified manufacturer's toggle switch for the Modular Receiver under a qualified toggle switch part number. Gems Sensors was unable to produce evidence that they performed the required analyses to ensure that the toggle switches being stocked were equivalent to the qualified toggle switches.

Gems Sensors Division Cowles Road Plainville, CT 06062-1198 Descriotion of the Problem Gems Sensors Division has failed to maintain design change control sufficiently to ensure the qualified integrity of the Model 63328A Modular Receiver.

Hyle Laboratories Qualification Report No. 45700-1 for the Model 63328A Modular Receivers indicated specific manufacturers, models, and catalog numbers for some components including toggle switch St. Revision K to the part number / drawing for toggle switch S1 lists multiple nonqualified manufacturer's toggle switches as equivalent to the qualified toggle switch; however, Gems Sensors could not produce evidence of a commercial grade dedication process for i

the nonqualified toggle switches.

The only toggle switch currently being i

supplied under the part number for Si is a nonqualified switch.

mucu mann SHUD. performed a vendor audit of the Gems Sensors Division of IMO Delaval during February 1988. During this audit an investigation of the equipment qualification traceability of a toggle switch-ordered by SHUD was performed.

The investigation determined that a nonqualified manufacturer's toggle switch was being stocked under the qualified toggle switch part number.

The qualified toggle switch is a Cutler-Hammer switch, 3PDT, part number 7670K and is identified as such in the bill of materials for Hyle Laboratories Test Report No. 45700-1.

The switch presently being stocked under the qualified toggle switch part number is a Carling Electric switch.

Gems Sensors could not produce evidence that they had performed the required analysis to ensure that the toggle switches being stocked were equivalent to the qualified toggle switches.

The SHUD auditor immediately notified the Gems Sensors Quality Assurance management of the potential reportability under 10 CFR 21 and advised them to take immediate action to evaluate reportability.

Since (to the best of SHUD's knowledge) Gems Sensors has not st.bmitted a 10 CFR 21 s eport on this item, SHUD is filing this 10 CFR 21 report to alert the industry to a potential problem.

Number and Location of Such Comoonents at Rancho Seco Rancho Seco has four Gems Sensors Model 63328A Modular Receivers.

The receivers are used for containment emergency sump level and Containment.

Building water level. All of the Gems Sensors Modular Recrivers at Rancho Seco have the qualified toggle switch installed; therefore, there was no safety significance to Rancho Seco due to this problem.

Corrective Action The immediate corrective action taken was to remove Gems Sensors from SHUD's Approved Suppliers List and to document the potential problem with the toggle switches.

Upon notification of the potential problem, each of the Hodular Receivers at Rancho Seco was inspected to determine which toggle switch was installed in the receivers.

Each of the receivers at Rancho Seco has the qualified toggle switch installed.
