Grand Gulf Nuclear Station Units I and 2 Docket Nos. 50-416 and 50-417 License No. NPF-29 File: 0260/0311/0512 Reportable Deficiency 85/03, Interim Report - MSIV Failure to Remain in Closed Position Due to Failure of the Dual Solenoid (ASCO) to Transfer When De-energized AECM-85/0096 On March 21, 1985, Mississippi Power & Light Company (MP&L) notified Mr.

Vince Panciera of your office of a Reportable Deficiency at Grand Gulf Nuclear Station (GGNS) Unit 1. The deficiency concerns the failure of dual solenoid valves to transfer when de-energized.

This deficiency was determined to be reportable under the provisions of 10CFR21 for Unit 1. Applicability to Unit 2 has not been deterrained at this time. Attached is an interim report on the subject. An update wLil he provided by April 30.

The Main Steam Isolation Valves were supplied to CGNS by General Electric Company, San Jose, California. The valve actuators were manufactured by Hiller-Sheffer Corporation and the solenoid valves which are installed on these actuators were supplied by the Automatic Switch Company (ASCO), Florham Park, New Jersey.

4. Nature of the deficiency . . . and the safety hazard which . . . could be created by such a deficiency . . ."

A. Description of the Deficiency On February 10, 1985, following a reactor scram, the Main Steam Isolation Valves (MSIVs) were manually slow closed in the " Test" position to limit the reactor cooldown rate. Following the slow closure procedure, the associated handswitches were placed in the

" CLOSED" position. At this point, three MSIVs (one inboard B21-F022D and two outboard B21-F028 A & C) re-opened.

One set of solenoid valves, single solenoid ASCO Model HTX832020V, is utilized to slowly close the MSIVs and the other set of solenoid valves, dual solenoid ASCO Model HTX832320V, which are the fast closure solenoid valves used in an accident, hold the MSIVs closed.

A preliminary analysis has attributed the cause of failure of the MSIV to close to the second set of solenoid valves, which failed to actuate when deenergized.

The purpose of the MSIVs is to isolate the Main Steam Lines to limit the release of radioactivity to less than the specifications of 10CFR100 in the event of a steam line break outside the primary containment.

It should be noted that only one MSlV in the three affected steam lines (A, C, & D) failed to remain closed. However, the failures have been attributed to the ASCO Model HTX832320V solenoid valves which failed to transfer when deenergized. Therefore, based on the redundant basic component guidelines, as defined in NUREG-0302, it is conservatively postulated that a failure of both MSIVs in a steam line could potentially exist. This failure coincident with a postulated Main Steam Line break outside the primary containment, and absence of operator action, could result inLa reduction in the degree of protection provided to public health-and safety.

5. The date on which the information of such deficiency . . . was obtained:

Mississippi Power & Light received information of the deficiency on February 10, 1985. An evaluation under the guidelines of 10CFR21 was concluded on March 20, 1985.

6. In the case of the basic component . . . the number and location of all such components:

There are eight fast close solenoids installed on the MSIVs.

7. The corrective action which has been taken . . . the name of the individual responsible for the action; and the length of time that has been . . . taken to complete the action:

A. Corrective Actions Taken

1. Material Nonconformance Report MNCR #0070-85 was issued to document the condition.
2. Tests are in progress by GE and ASCO to determine the cause of

.the solenoid valves failure to transfer when deenergized.- To date the tests have been inconclusive. MP&L elected to replace the HTX-series valves with NP-series.

3. All eight fast close solenoid valves have *een replaced with ASCO NP-series type solenoid valves, per Design Change Package DCP-84/3084.
4. Further corrective actions will depend on the GE and ASCO test results.
