Dear Mr. Denise:

This letter provides the final report submitted pursuant to 10CFR50.55(e) concerning Excore Neutron Detector field cabling at Wolf Creek Generating Station (WCGS) . This matter was initially reported by Mr. H. K. Chernoff of Kansas Gas and Electric Company (KG&E) to Mr. Robert Taylor of the Nuclear Regulatory Commission, Region IV, on October 9,1984.

During an inspection of two excore neutron detector wells (SE-02 and SE-06) the " field run" triaxial cables, which connect to the detector pigtails, were found damaged apparently as a result of excessive heating. Design drawings indicate that the entire triaxial cable length (including pigtail connectors) should be placed within the conduit which leads to the wall.

Contrary to this, observations revealed that the triaxial cables extended into the detector well space, outside of the conduit. Through further observations the Architect / Engineer (A/E) identified that several removable insulation panels had been removed and never replaced. As a result of this information the A/E has determined that the aforementioned conditions were directly responsible for the triaxial cable damage.

In order to correct the discovered condition, all damaged triaxial cables and connectors will be repaired and/or replaced. Additionally, all insula-tion panels will be verified to be properly reinstalled.

The aforementioned corrective actions will be completed prior to fuel load.
