Pacific Scientific Company was notified by Wolf Creek nuclear operating corporation that !

during testing (at 20% of rated load) Pacific Scientific Company Part number 1801533 03 i Rod and bearing assemblies were falling at the mechanical swagejoint between ,

the plug end that contains the spherical bearing.  !

A representative of Wolf Creek hand carried some of these parts to our Yorba Lind where we tested 4 of them in the presence of their representative. Testing confirmed !!

premature failure. Further investigation of manufacturing documents, at Pacific Scientific Co. Indicates 20 parts were manufactured for Wolf Creek and did not contain the!; ,

test data to substantiate adequate testing was performed. Wolf Creek has notified us tha all 20 assemblies are accounted for and any which had been in service had been re:

A report will~be issued to Wolf Creek by Pacific Scientific Co. outlining root causel Corrective actions to prevent recurrance. i An additional 100 parts from an order for.110 were. manufactured in approx same time frame for Philadelphia Electric Peachbottom plant. Review of mf i documentation revealed lack of required test data but did contain inspection technition.

We strongly suspect Philadelphia Electric parts comply with required load requirements however they have been notified to return several to us for t confirm compliance. All 100 parts shipped to Philadelphia Electric are in th all in stores and none in service.

NRC will be advised of test results of Philadelphia Electric parts.

Review of manufacturing documentation for orders to be shipped to other utilities all tests as required were performed and test data available. Evidence indic defective parts these allin possession of Wolf Creek.

Your stock code number PECO 115 62435 our part number 1801533 03 Rod & Bearing  :

assembly was manufactured approximately in the same time frame as similar items manufactured for Wolf Creek which exhibited premature failure during load testing. Failure ,

was caused by improper staking of bearing rod end to rods and improper or lack of testing during manufacture to assure load bearing capability of the assembly.

We manufactured 110 of these assemblies on our manufacturing outline shoporder No.1-1905-558150 (copy enclosed) operation 0020 requires test to failure of first part staked and i recording of test load at fnilure on the shop order. Test load was not recorded as required only Functional test stamp Q4 dated 10/12/87 as evidence of test in addition to Quantity  !

to Stores was 109 indicating loss one assembly from lot of 110.

We are fairly confident the parts supplied to you are satisfactory for use however we wish to be positively sure by reviewing and possibly testing to failure 2 or 3 of the assemblies now ,

in your possession.

Enclosed is a copy of the shop order which manufactured your parts and a copy of PSCO 01.07 which describes the staking and test procedures.
