Special Report 98-001 l

) Inoperable Waste Gas System Oxygen Analyzer This Special Report is being submitted pursuant to the requirements of Updated Safety Analysis Report (USAR) section 16. 3.1. 6 and Technical Specification 6. 9.2. USAR section 16.3.1.6 states that the explosive gas monitoring instrumentation channels shown in table 16.3-7 shall be operable with their Alarm / Trip Setpoint set to ensure that the limits of section 16.11.2 are not exceeded. USAR 16.3.1.6 Action Statement "b" requires a special report to the NRC if operability is not restored within 30 days.

On June 14, 1998, the Radwaste Operator observed that the waste gas system oxygen analyzer OARC-1119A was indicating 0 ppm. The ir.itial reading remained constant for approximately two and one half hours. On June 15, 1998, the Radwaste Operator observed that the readings were fluctuating between 200 to 900 ppm. The oxygen analyzer on the waste gas system was declared inoperable at 0100 CDT on June 15, 1998. Entry 98-188 was made to the Equipment Out of Service Log (EOL) and an entry was also made in the action statement summary log. Work Request 98-010439 was generated, noting the 30 day requirement of USAR 16.3.1.6, Action Statement "b". However, the troubleshooting package did not contain any reference to the USAR 30 day limit, so it was decided Work Order 98-201246-000 would be i worked with STS IC-470A, " Gaseous Radwaste H2 and 02 Monitors HA-161 Train A - Channel /

Calibration /ACOT", scheduled for July 15, 1998.

On July 15, 1998, Work Order 98-201246-000 was completed in conjunction with the performance of STS IC-470A, and OARC-1119A was declared operable at 1557 CDT, 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> and 57 minutes past the 30 day reporting requirement. The sampiing requirements of USAR table 16.3-7 Action Statement "2" were met during the period CARC-1119A was inoperable.

The inoperability of OARC-1119A was not corrected in a timely manner due to personnel error in that the 30 day time requirement was not documented on work package documents, i Immediate actions were taken to notify appropriate personnel of this event and remind them I of the need to review and document time requirements. Performance Improvement Request 98-2040 was issued to document the evaluation and corrective action associated with this ,
