This letter is being submitted pursuant to 10 CPR 21.21(b)(2) as a follow-up to a telephone call made on March 29, 1988 by O. L. Maynard, Wolf Creek Nuclear Operating Corporation, to D. Carpenter and J. Tapia, NRC Region IV.

The components which are the subject of this notification are Class IE fuses which were procured from the following company:

Planned Maintenance Systems 1002-4 Main Street Mt. Vernon, IL 62864 One of the requirements of the purchase order issued to Plannad Maintenance Systems (PMS) was that the items specified in the purchase order were to be supplied in accordance with the requirements of a specific fuse qualification specification for Class IE equipment. This specification contained detailed requirements including materials, environmental qualification, seismic qualification, and inspection / test requirements.

When the fuses were received, the Certificate of Compliance (C of C) supplied with this order certified -

'st all the procurement document requirements had been met and no ceviations from the requirements were identified. However, the fuses were not accepted for release by the receiving organization but were put on "Hold" because of another purchase order requirement which required a Quality Department surveillanca at the FMS facility.

WM 88-0086 l- Page 2 of 2 April 1 , 1988 During the conduct of this surveillance at the PMS facility, it was found that the fuses were procured by PMS from Gould, Inc. through a distributor, National Electric Service Company. A review of the documentation at PMS showed that National Electric Service received the fuses from Gould and then '

direct shipped them to PMS along with the could C of C.

The surveillance revealed that the QA records in PMS's possession did not support the PMS C of C statement that all the purchase order requirements had been met. Specifically the fuses that were shipped did not meet an inspection requirement consisting of a continuity / resistance check of each fuse prior to shipment. Also, 'certain qualification specification requirements were not covered by PMS QA records such as environmental qualification radiation levels and seismic qualification information.

In addition, the eurveillance inspector noted some inconsistancies in the form and content of the Gould C of C in PMS's possession. Upon return from PMS, a follow-up was conducted by WCN00 to provide assurance that the Gould C of C reviewed at PMS was reflective of the certification activities performed by Gould. Problems were encountered in this follow-up activity in that Gould stated they did not perform some of the certification activities that were attributed to Gould on the C of C in PMS's posaession. This discrepancy has not yet been resolved.

A total of 60 fuses were ordered / shipped from PMS. As stated above, these l fuses were not accepted but were put on "Hold". They remain on "Hold" in l

the receiving warehouse. None have been released from the warehouse or '

l installed. This was the first and only purchase order issued to PMS.  ;

Subsequent follow-up activities included a review of past and present procurement documentation at WCN00. It was verified that no other orders ,

had been placed with PMS .and also that no orders had been placed with l

National Electric Service Company.
