Subject: NOTICE OF VIOLATION (VERMONT YANKEE INSPECTION 93-26) ,

This refers to the safety inspection conducted by Messrs. H. Eichenholz and P. Harris on

'

October 10 - November 27, 1993 at the Vermont Yankee Nuclear Power Station, Vernon,

Vermont. The preliminary results were discussed with Mr. R. Wanczyk at the conclusion of  :

the inspection. The inspection focused on issues important to public health and safety, and

consisted of performance observations of the conduct of operations and maintenance, and-

independent evaluation of safety system status and quality records.

Good performance was demonstrated by your operating staff during startup and surveillance

activities. However, we were concerned that the lack of administrative controls for certain '

activities resulted in the unintended modification of a plant structure and inadequate control of -

scaffolding within safety-related areas of the plant. Specifically, polyethylene plastic was

installed for a period of approximately two weeks over a hatchway within the reactor building

that potentially modified environmental qualification conditions, and scaffolding was not properly ,

installed nor evaluated to assure the continued operability of safety systems during a seismic

event. These instances represent recurrent examples of recent concerns regarding work control, ,

and together constitute a violation of Technical Specification requirements, as specified in the

enclosed Notice of Violation.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practices," a copy of this letter and '

the enclosed Notice will be placed in the NRC Public Document Room. You are required to

respond to this letter and should follow the instructions specified in the enclosed Notice. The  !

responses directed by this letter and the enclosed Notice are not subject to the clearance .i

procedures of the Office of Management and Budget as required by the Paperwork Reduction  ;

Act of 1980, Public Law No. 96.511.  !
