SUMMARY

- V0GTLE ELECTRIC GENERATING PLANT - DOCKET NO. 50-424 This letter refers to the meeting conducted at the Vogtle site on June 30, 1987. This meeting was held to discuss the corrective actions being taken to reduce the high reactor trip rate and address other programmatic problems.

Our concerns regarding identification of root cause and achieving effective corrective action, learning from other utility experience and improving the teamwork of your staff expressed to you at the meeting and your positive response to our concerns is appreciated.

Currently, a followup meeting is being scheduled for early September to monitor your progress.

It is our opinion that this meeting was beneficial and has provided for a better mutual understanding of the inspection findings and enforcement issues.

We also found your description of the findings and your corrective actions to be beneficial in our evaluation of the issues. In accordance with Section 2.790 of NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the enclosures will be placed in NRC's Public Document Room.

The Operations Supervisor initiated an emergency work order to repair the valve. The MSIV contains a high pressure volume of nitrogen which forces the valve toward the closed position. Two equivalent hydraulic operator systems hold

-he valve open against this pressure. l The operations Supervisor told the Maintenance Foreman to  !

" block" the valve operator before proceeding. To the

{

Operations Supervisor this meant to isolate, or block, the hydraulic fluid path. To the Maintenance Foreman it meant something else. To " block" the valve he placed lengths of 3" pipe to prevent mechanical closure of the MSIV. He had done this previously during system flushing evolutions. The faulty solenoid valve was first isolated then replaced in the fluid pathway. To connect the electrical wiring, Operations wrote a clearance and a fuse was pulled. This I removed power to both hydraulic operator systems. The unisolated system allowed the Nitrogen pressure (not presen*

during flushing test) to close the valve, bending the pipe.

The plant tripped when steam generator shrink resulted in a Lo-lo level signal.

throttle pressure limiter which generated a signal to close the turbine control valves.

RESULTING SITUATION I Control valves began to close and generator output dropped to 80 MWe, RESPONSE TAKEN The B0P operator quickly recognized the problem and immediately switched over to standby turbine control.

RESULTING ACTION Due to this quick response, generator output was stabilized and transient was halted, restored to 500 MWe.

WHAT COULD HAVE HAPPENED?

During transient, the steam generator levels rapidly decreased to less than the 40% level which could have led to a reactor trip if prompt action had not been taken.
