OF-MAY 23-25, 1989 MEETING AT V0GTLE U UNIT 2 - REGARDING THE MARCH.1989 CHECK VALVE LEAK- 1 An onsite meeting was held on Ma with representatives of '

the' Georgia- Power Company and (GPC) y 23 thru members of the25,1989, Human Factors Assessment Branch and Reactor Systems Branch of NRR to review the Yogtle Unit 2 response to the March 1989 ECCS check valve leaks.  ;

Discussions and interviews were held between the NRC staff and GPC personnel ,

from operations, training, licensing, quality assurance and management involved i with the circumstances surrounding the leakage of reactor coolant to the reactor water storage tank (RWST) through the residual heat removal system (RHR). The NRC staff was primarily interested in learning what human performance 1 factors. contributed to the event and to develop a better understanding of the 1 precursors and predictors of intersystem LOCAs (ISLOCA). 1 l

The NRC' staff focuse'd on several aspects of the Vogtle Unit 2 check . valve leak ]

event that will contribute to the development of a pilot inspection program for 9 ISLOCAs. These pilot inspections will be performed to assess the sources of l ISLOCAs and determine how vulnerable power reactors are to ISLOCAs. Interpersonal 2 communications, plant systems and equipment status awareness, procedures and -

training were discussed during the interviews. In addition, document reviews, control room, simulator, and plant walkdowns were performed.

At the conclusion of the meeting, the NRC staff expressed their appreciation to GPC management for.the time and cooperation of GPC personnel that arranged and ,

coordinated the meeting and provided the docunents and access to the plant and 4 training facilities.
