APPENDIX A NOTICE OF VIOLATION Louisiana Power

' During an NRC inspertion conducted September 17 through October 31, 1988, a violation of NRC requirements was identified. The violation involved failure to follow procedures. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violetion is listed below:

Failure to Follow Procedures TS 5.8.1.a requires, i.. part, written procedures to be implemented for operation of safety-related systems as recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

) and Breakers," paragraph 5.1.2.1, requires documented control of locked valves such as SI-2421 (Safety Injection to Loop 1B Containment Isolation Check Valve SI-242 Bypass) status. Attachment 6.2 of OP-100-009 requires SI-2421 to be locked clor:d.

) Contrary to the above, on October 25, 1988, the NRC inspectors found

SI-2421 closed but not locked. There was no documentation allowing this '

deviation from the required status nor was there any procedu.e in progress that would require the valve to be unlocked. The valve could have been ,

unlocked as early as April 1988 when the last valve status verification was performed.

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power and Light is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted,  ;

(2 the corrective steps which have been taken and the results achieved, <

t (3 the corrective steps which will be taker. to avoid further violations, and (4 the date when full compliance will be ach1eved. Where good cause is shown, '.
