APPENDIX A NOTICE OF VIOLATION Public Service Company of Colorado Docket: 50-267/88-09 Fort St.'Vrain Nuclear Generating Station Operating License: DPR-34 D'u ring 'an NRC inspection conducted on April' 11-15,~ 1988, a violation'of NRC requirements was identified. The violation involved the' failure to properly evaluate. water chemistry instrument quality control data. In accordance with the."General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1987), the violation is listed below:

Water Chemistry Instrument Quality Control Charts Technical Specification AC 7.4 states, "Written procedures shall be established, implemented, and maintained covering the activities referenced below:

1. The applicable' procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972."

The licensee's Water Chemistry Procedure-356, "Analytical Control Charts,"

Issue 1, dated October 21, 1987, paragraph 5.8.3 requires that, when an out-of-control situation occurs, analysis must be stopped until the problem has been identified and. resolved, after which the frequency should be increased for the next few quality control checks and the problem and solutions must be documented. Paragraphs 5,8.4 and 5.8.5-further define corrective actions to be taken following identification of out-of-control situations which include the making of new instrument control charts utilizing new standards, duplicates, and spikes and all analyses since the last in-control point must be evaluated and appropriate action taken.

Contrary to the above, the NRC inspectors determined on April 13, 1988, that the water chemistry instrument quality control charts for analyses for chloride by ion chromatography, silica by spectroscopy, and iron and copper by graphite furnace atomic absorption had not been properly evaluated and several examples of out-of-control situations existed for each of the above analyses during the time period November 11, 1987,.

through April 13, 1988, with no documented evaluation or corrective action taken as directed by procedure.

is--hereby required to submit to this office, within 30 days of-the date of the letter transmitting this Notice, a written statement or explanation in reply, including for'each violation: (1) the reason for the violation if admitted, (2) the corrective steps'which have been-taken and the results achieved,

- (3) the. corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given.to extending the response time.
