! ' On February 7,1997, the NRC completed an inspection at your Formi 2 reactor facility.

Onsite inspections were conducted during the periods of December 2-6,1996'and

January 13-24,1997. The enclosed report presents the results of that inspection.

i- Areas examined during the inspection are identified in the report. Within these areas, the

I inspection consisted of a selective examination of procedures and representative records,  ;

interviews with a large number of personnel, and observation of activities in progress. The

l ,

objective of the inspection effort was to determine whether activities authorized by the j

license were conducted safely and in accordance with NRC requirements. The summary

j i

contained in the report provides an overview of our assessment of your security program  !

l performance.

j Based on the results of this inspection, the NRC has determined that a violation of NRC i

! requirements occurred. The violation is cited in the enclosed Notice of Violation (Notice) ,

j

and the circumstances pertaining to the violation are described in detail in the enclosed

.

' I

!

report. The violation is of concern because it was identified by the NRC and demonstrated  !

I lack of attention to detail by your security staff in complying with a requirement to obtain

personal consent prior to initiating background investigation activities. This violation is

i

being cited because it was not identified through self assessments.

The NRC has concluded that infonnation regarding the reason for the violation, the

.

corrective actions taken and planned to correct the violation and prevent recurrence is  !

i

addressed in Section S5 of the Report Details. Therefore, you are not required to respond

!

l to the Notice of Violation unless the information in the Notice and Section S5 of the j

Report Details does not accurately reflect your corrective actions or your position, or if you "

choose to provide additional information.

l. Self-identified issues such as poor security management credibility, high security force  !

turnover rate, continued adverse performance trends, and communication weaknesses

!

were confirmed during our inspection effort and represented a significant challenge to the

j security force management team,
