I FINAL REPORT OF SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 106

" GULF ALLOY, INC., 2" FPT x 1 " BUSHING F-304" INTRODUCTION This report is submitted pursuant to 10CFR50.55(e). It describes bushing material purchased from Gulf Alloy, Inc., which does not meet the requirements of ASME NCA-3800.

This deficiency has been reported to the USNRC pursuant to 10CFR21, by the material manufacturer Bonney Forge Carlinville Plant.

DESCRIPTION Gulf Alloy, Inc., supplied the Waterford #3 site with twenty (20) fittings (2" FPT x 1 " bushings) on purchase order number WP3-4336. In a letter dated 3/26/84, Gulf Alloy, Inc., notified Ebasco that the material manufactured by Bonney Forge Carlinville Plant was lacking the overcheck requirement of paragraph 3867.4(e)(2).

These fittings were traceable through the heat number and purchase order to the Ebasco warehouse.

SAFETY IMPLICATIONS The material in question was not installed. Therefore, no safety implications exist.

CORRECTIVE ACTION NCR-W3-7652 was issued to scrap the (twenty) 20 fittings.

Corrective action was completed on April 6, 1984, and the supporting documentation has been reviewed and the NCR was closed on April 6, 1984.
