An additional concern is the steam system piping failures have the potential to release significant amounts of radioactive material. This release in radioactive material, caused by primary-secondary leakage, presents a safety hazard.

NCR-W3-3206 - The same potential problem described in NCR-W3-3202 exists i in this case. Failure could reduce feedwater flow to the steam generator, l causing a reactor coolant system heatup. This RCS heatup has the potential i to cause core damage. Therefore, if the defective welds are not properly l removed, the safety of the plant could be adversely affected.

CORRECTIVE ACTION TAKEN The " Unapproved Welding" per this Significant Construction Deficiency was identified, dispositioned and corrected per the three (3) following Non-conformance Reports:

1. NCR-W3-2971, Rev. 1: The twelve (12) attachment welds were removed and rewelded with approved procedures and personnel. The welds were examined and found acceptable. The NCR was reviewed, accepted and closed December 7,1982.
2. NCR-W3-3202: The one (1) temporary attachment weld was removed and the base metal was repaired, examined and found acceptable. The NCR was reviewed, accepted and closed September 25, 1982.
3. NCR-W3-3206: The or.e (1) temporary attachment weld was removed and the base metal was repaired, examined and found acceptable. The NCR was reviewed, accepted and closed October 1, 1982.

This report is submitted as the Fiaal Report.
