Dear Mr. Seidle:

In accordance with 10 CFR 50.55(e) we are submitting this progress report concerning identified deficiencies as a result of unauthorized field modifications of Hilti-Kwik concrete anchor bolts during the installation process. We have previously reported this item to your Mr. R. G. Taylor on October 1, 1980, and hid submitted an interim report to you forwarded by TXX-3223 dated October 21, 1980.

The initial phase of the test program utilizing ultrasonic test methods was completed on December 31, 1980. This program was established to test and evaluate each functional craft discipline / subcontract which installed Hilti-Kwik anchor bolts. The craft disciplines / subcontracts evaluated were electrical, fire protection, HVAC, instrumentation and mechanical.

Based on the results of the initial phase of the test program, there is sufficient assurance that the mechanical, instrumentation and fire protection Hilti-Kwik installations have not been modified in an un-authorized manner. Therefore, no additional testing in these areas is anticipated at this time. Certain anomalies were detected with i
