NATURE OF THE DEFECT OR FAILURE TO COMPLY AND THE SAFETY HAZARD WHICH IS CREATED OR COULD BE CREATED BY SUCH DEFECT OR FAILURE TO COMPLY:

A Borg-Warner swing check valve failed to fully close during pre-operational testing. The valve was ,

radiographed and the disk was found to be lodged in the full open position. Subsequent disassembly revealed four '

points of contact between the disk-swing arm assembly and the body. The contact points occurred at the junction of the two machined bores perpendicular to .the flow direction. All other Borg-Warner valves tested at the  ;

utility have functioned properly. Proper function was also supported by the results of acoustic monitoring.

Several of these valves exhibited similar evident of  ;

contact but functioned properly.

Failure to close is attributable to the internal body wall protrusions which result from the intersecting machine bores. ,

THE DATE ON WHICH THE INFORMATION OF SUCH DEFECT OR FAILURE TO COMPLY WAS OBTAINED:

The incident was initially reported to BW/IP on 18 December 1992. Additional information has subsequently been supplied by Texas Utilities.

IN THE CASE OF A BASIC COMPONENT WHICH CONTAINS A DEFECT OR FAILS TO COMPLY, THE NUMBER.AND LOCATION OF ALL SUCH COMPONENTS IN USE AT, SUPPLIED FOR, OR BEING SUPPLIED FOR ONE OR MORE FACILITIES OF ACTIVITIES SUBJECT TO THE REGULATIONS:

This notice appl 3-; to all Borg-Warner 4, 6, 8, and 10-inch, 150 and 3i 0#, bolted bonnet swing- check valves ,

which have a two pace forged body construction.

THE CORRECTIVE ACTION WHICH HAS BEEN, IS BEING, OR WILL BE TAKEN: ,

ACTION: AND THE LENGTH OF TIME THAT HAS BEEN OR WILL BE TAKEN TO COMPLETE THE ACTION: 5 Corrective actions are:

Inspect installed valves for evidence of disk-body contact at the protrusions. If evidence of contact is observed, protrusions should be removed and blended to  ;

the internal body contour.

For new valve applications, design modifications will be  !

implemented to assure proper operating clearances between the disk and valve body. i I
