Dear Mr. Burrer:

This letter is official notification in accordance with 10CFR21.21 of a deviation to specification. Computer Products, Inc. (CPI) has identified that the RTP Universal Input / output Controller Interface Card utilized in.the Universal I/O Controller (UIOC), which is supplied for installation into U.S. Nuclear Generating facilities, does not meet elevated operating temperature specifications as published.

Wo have determined that the affected revision has been supplied to SAIC for installation at Texas Utilities Electric - Comanche Peak.

Shipping history is as fo' lows:

Model #: NEQ7431/60-002 - Shipped quantity 4, September Shipped quantity 1, April 1992. Shipped 1991.

quantity-4, October 1992. Total quantity 9.

The deviation is caused by increased component susceptibility, at ambient temperatures above 90"F, to circuit anomalies. The effect of the anomalies are on a component of.the 021-5282-000 circuit board sub-assembly. These effects are attributed to the circuit layout of the 021-5282-000 Egvision E, circuit board.

The circuit board sub-assembly is a interface-card which plugs into the NEQ7431/60-002 UIOC chassis. . The suspect units can be identified by Computer Products part number NEQO21-5282-000 Eevision E only.

This deviation does not affect the normal operation of the circuit cards or controller chassis; however, in a environment where temperatures exceed 90 F, these units will not perform as specified.

Model Number Breakdown Model # NEQ7431/60-002 consists of part #'s:

NEQ070-5089-002 - Chassis, Universal Input / Output Controller (UIOC)

NEQO21-5282-000 Rev. E - Circuit Card, UIOC Interface Card The design has been corrected through a Engineering Change Order (ECO) adding two filter capacitors. These capacitors slow signal rise times preventing the effects of circuit anomalies which cause the deviation. The ECO changed the revision of the 021-5282-000 to revision E1. j Computer Products, Inc., as a supplier, does not feel that we have the capability to perform an evaluation as to the effect this deviation may have regarding whether or not a substantial  ;

safety hazard exists.

is necessary and will require shutdown and removal of the circuit j board sub-assembly from the UIOC chassis and system.

In accordance with 10CFR21.21, Computer Products, Inc. is simultaneously notifying the Nuclear Regulatory Commission and Texas Utilities of this deviation.
