- LAttachmen't-to TXX-92380-Page 1 of 1 10CFR50.55(e) REPORTABLE DEFECT CONCERNING CRACKED WELOS IN CONTAINHENT SPRAY PUHP SUCTION VENT PIPING (i)
Information supplied by:
i William J; Cahirl, Jr.
TU Electric 400 North Olive Street, L. B. 81 Dallas, Texas. 75201 (ii)
The basic component is a 3/4' stainless steel vent pipe on the
. suction of the Train "A" containment. spray pump.
Lfiii)
The f acility is being constructed by Brown & Root for TV Electric.
(iv)
.Two welds on a-3/4" stainless steel vent p*pe for the containment spray' pump suction were found cracked during system flushing.
The welds had passed post weld QC inspection and system hydrostatic testing.
During the recirculation phast of a loss of Coolant Accident, the vent-line~ could contain radioactive liquid. f rom the containment sump.
This~liquio ;ould be released in the containment spray pump coom representing a serious con, romise of the ability of a radiction confinement system to perform its designed function.
(v)
This defect was identified and documeited on June 14, 1992.
J (vi)
No other. defects of this type have been found.
=(vii)
The affected section of piping was removed for evaluation and replaced with new pipe and fittings.
The welds have been-*9nt to
- a-laboratory to determine the cause of cracking.
The ' history.of the welder who made both welds will be collect?d and.a radiographic reject' rate analysis will be performed to determine if further evaluation of the welder's wo-k is required.
- Corrective actions will be complete prior to Unit 2 fuel load.
-(viii)
CPSES Unit I was' informed ~of the defect.
