YOGTLE ELECTRIC GENERATING PLANT TECHNICAL SPECIFICATION ' IMPROVEMENT PROGRAM Requirement 3 of the Technical Specification Improvement Program in Section 16.3 of the Final Safety Analysis Report (FSAR) requires that, with one or more of the loose part detection system channel (s) inoperable for more than thirty (30) days, a special report shall be prepared and submitted to the Comission within the next 10 days. This report shall outline the cause of the malfunction and shall describe the actions required to restore the channel (s) to operable status.

Enclosed is a special report concerning the failure to perfom a channel calibration of the loose part detection system.

L00SE PART DETECTION $_YSTEM A. REQUIREMENT FOR REPORT This report is submitted in accordance with Vogtle Electric Generating Plant's Final Safety Analysis Report (FSAR), Section 16.3, Requirement 3. l This section requires a spects1 report to be submitted to the Comission  :

within ten days when one or more of the Loose Part Detection System j chennels are inoperable for more than 30 days.

B. DESCRIPTION OF LOOSE PART DETECTION SYSTEM INOPERABILITY I On October 16, 1989, plant perr.onnel discovered that the channel  ;

calibration for the Loose Part Detection System was not performed as l specified in the FSAR, Section 16.3, Requirement 3. A Maintenance Work l Order (MWO) generated under the Preventive Maintenance (PM) Program was  !

initiated on July 27, 1988, to perfom the 18-month channel calibration i during the first refueling outage for Unit 1. The MWO was erroneously f voided due to the belief it was not required to be included in the scope of the refueling outage. The FSAR comitment to demonstrate the i

operability of the Loose Part Detection System was not properly identified '

in the PM progrant which resulted in the MWO not being worked during the outage.

For corrective action, the channel calibration for the Loose Part  !

Detection System has been entered into the Technical Specification l Surveillance Tracking Program. Since this surveillance can only be l perfomed in Mode 5, the operability of the Loose Part Detection System (

will be demonstrated during the next refueling outage for Unit 1. l Additionally, it should be noted a channel check has been performed at  !

least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> and an analog channel operational test has been performed at least once per 31 days in accordance with FSAR Section 16.3.  ;
