Addendum to Reply to Notice of Violation-528/89-16-01, 528/89-16-03, 528/89-16-04, 530/89-16-01, 530/89-16-03 File: 89-0 0-026 The referenced letter stated that APS had conducted tests to determine the actual flow conditions in the Reactor Coolant System (RCS) and Shutdown Cooling (SDC) systems. Additionally, the results of the testing were to be incorporated into the appropriate operating procedures.

This testing has been conducted in Units 1 and 3. This testing will be performed in Unit 2 during the upcoming refueling outage. APS is also performing studies using a scale model of the Palo Verde RCS to gather further data on vortex phenomena. The engineering analysis of the data gathered during the above testing is expected to be completed by May 31, 1990, and the appropriate operating procedures will be revised if ncessary by June 30, 1990.

In the interim, the Unit 1, 2, and 3 operating procedures, Shutdown Cooling Initiation (4XOP-XSIOl, X 1, 2, and 3 for the three units) have been revised to minimize air entrainment by providing the following guidance:

1) Only one SDC pump and only one SDC loop may be in operation when in a mid-loop condition (i.e., RCS level less than the 103 feet 1 inch elevation),
2) Time that the RCS level is below the 104 ft. elevation shall be minimized,

3) The SDC loop will be periodically vented during operation to remove accumulated gas/air from system high points.

Additionally,,operating procedures have been written to specifically .address "RCS Drain Operations" (410P-1ZZ16 and 430P-3ZZ16), which includes items 1 and 2 listed above, and provides the following guidance:

SDC flow shall be maintained between 4000 and, 4100 gpm (this is the smallest useable band available based on the Technical Specification minimum flow rate of 4000 gpm).

Based on the testing performed in Units 1 and 3, APS believes that this guidance is sufficient to prevent a loss of SDC event due to air binding the SDC pumps as a result of vortexing/air entrainment.

The Unit 2 operating procedure, RCS Drain Operations (420P-2ZZ16), will be issued prior to mid-loop operations in Unit 2.
