Huey, of your staff, in a phone call held on June 21, 1990, concerning the information contained in Reference 1, APS is providing an update to the failure data information which was originally provided to the NRC in -the meeting held on June 14, 1990. The updated failure data information is provided in Attachment 1. References 2 and 3 provided, also as requested in the phone call, a detailed reliability evaluation of the Exide Power Supplies.

Ln addition to this information, replacement pages are provided for 'Reference 1 and Reference 2 in Attachments 2 and 3, respectively. These replacement pages provide changes which are editorial in nature.

ATTACHMENT 1 4 I EMERGENCY LIGHTINGSYSTEMS QB AND QD FAILURE ANALYSIS BASIS

1. This analysis is a compilation of known QB and QD failures from the plant Failure Data Trending System (FDT), Station Information Management System (SIMS), and Material

.Management Information System (MMIS). There may be other failures unrecorded or improperly recorded that were not picked up by this analysis. A failure record represented in this analysis does not necessarily imply the inability of the component to perform the 8 hour9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> illumination requirement. Incipient failures identified during re-evaluation were not:added to the Emergency Light Failures list. This represents data from May 1987 (Work Order closed date) to May 1990 (work completed date). The exception,.is for those failures identified during this time frame which were prior to Unit 3 Commercial Operation date. (January, 1988)

2. The raw data is presented by Unit and building location per the-legend on page 2. The data was then collated and presented on plant maps by unit, building, and year and by manufacturer, unit, building, and year. Component populations contained'in pages 23 through 26 reflect the recently revised as built drawings which were verified by walkdown. Additional Holophane failure data is included for the areas not provided (or requested) in the 161'-03288 WFC/RAB-06/19/90 response. (All of Unit 2 and QBN001 AND QBN003 in Units 1 and 3.)
3. The organizations providing data were: Quality Assurance, which submitted all items annotated with a "QA" source code and a portion of the "SMS" annotated data; the Engineering Evaluations Department, which submitted'all items annotated as "MMIS"'and a portion of the "SIMS" items; and Nuclear Engineering which submitted all.items labeled as "FDT" and the remainder of the "SIMS" source data.
4. The assigned dates found on the emergency lighting failure data are the work document start date, the work completion date (i.'e. the date maintenance. on the equipment / component was completed), or the material issue date, depending on the system the data came from.

Specifically:

a. Failures discovered through the "FDT" are listed by the failure discovery date.
b. Failures from the "SIMS" database, depending upon. the organization that reviewed the data, used two different dates to indicate the time that the failure occurred Nuclear Engineering used the. work completion date as well as the failure discovery date if the information was still available on line in SINS. Both the Engineering Evaluations and the Quality Assurance Departments used only the work completion date.
c. Failures from '"MMIS" are listed by the date the material was issued from the warehouse.
d. The items marked "QA" are listed by. the work complete date.
5. Failures coded on the list as charge card / charger, bulb, or battery are recorded on the failure maps.

EXIDE UNINTERRUPTIBLE POWER SUPPLY RELIABILITY EVALUATION PVNGS UNITS' 2 AND 3 APS has performed an evaluation of the Exide Uninterruptible Power Supplies (UPS) to assess their ability to provide reliable lighting should'hey be called upon to do so. This evaluation was performed'y reviewing the current condition of each UPS, the result of the most recent eight hour discharge test, and the list of component failures for each UPS that was provided .to your staff in a meeting on June 14, 1990. This evaluation is summarized in this attachment along with a brief description of the .equipment and the maintenance program that is currently in place.

DESCRIPTION OF EXIDE UNINTERRUPTIBLE POWER SUPPLY The Exide UPS is made up of: a charger, twenty batteries (three cells each),

an inverter, an automatic transfer switch, and a minimum voltage, disconnect rel'ay. The UPS is designed to supply power to the emergency fluorescent lighting in the Control Room. The Control Room lighting is normally fed from the Class 1E Buses through regulating transformers. The automatic transfer switch will transfer from the regulating transformer 'to battery power when power is lost to i'ts respective bus and will transfer back when restored by either the Emergency Diesel Generator or restoration of off-site power.

The UPS consists of twenty wet cell batteries (6 volt) with three cells each, for a total of 60 cells. These twenty batteries are connected in a series configuration to supply 120 VDC to the Exide inverter which will supply 118 VAC to the fluorescent lighting in the Control Room. The UPS is also equipped with a constant voltage charger which is adjustable to allow for either float charging or equalize charging. The UPS also has a minimum voltage disconnect relay .which operates to disconnect the batteries from the load at 87.5% of nominal battery voltage. This, prevents the batteries from operating in a, deep- discharge condition.

The current loads and the available capacity at minimum operating temperature for each Exide UPS is summarized in the table below:
