Inspection on December 30, 1986 through January 14, 1987 (Reports

No. 50-295/66030(DRS); 50-304/86030(DRS))

Areas Inspected: Special announced safety inspection of environmental

qualification concerns regarding unqualified splice / cable assemblies

associated with cubicle cooler fan motors (Modules No. 30703 and No. 25176).

Results: No violations of NRC requirer,ents were identified; however, an

ongoing review is being performed to evaluate one EQ finding in regard to

possible future enforcement action.

2. Review of Splice /Cablo Assemblies

On December 8, 1986 the licensee notified the Zion NRC resident inspector

of Environmental Qualification (EQ) concerns regarding cubicle cooler

fan motor lead splice / cable assemblies used in 10 CFR 50.49 designated

applications. These lead assemblies were spliced with various types of

non-EQ tapes and sections of non-EQ PVC cable, and during this review

the licensee confirmed that they could not provide adequate EQ documenta-

tion to qualify these splice / cable assemblies to requirements of

10 CFR 50.49. In accordance with the guidance in Generic Letter 85-15,

the NRC inspectors informed the licensee that they were required to

provide a justification for the continued operation (JCO) of Unit 2, as

well as implement appropriate corrective action in Unit 1 prior to its

start up.

a. Justification for Continued Operation

During this inspection, the NRC inspectors reviewed the licensees

evaluation of the effects of postulated failures of the cubicle

cooler fan mctors on the operability of the plant, and reviewed the

licensee's scheduled corrective action.

Based on their evaluation, the licensee stated that the unqualified

splice / cable assemblies would only be exposed to a harsh radiation

environment during accident conditions in the containment. The

licensee also stated that previous radiation and aging tests

conducted by Wyle Laboratories (Report No. 45307-1, dated

December 23, 1981) and the Electric Power Research Institute

(Report No. NP2129) on PVC cable yielded acceptable results in that

there were no anomalies. In addition to the above, the licensee

sub_sequently inspected 18 motor lead splice / cable assemblies

in Unit I and found no indication of degradation. The NRC

inspectors informed the licensee that additional inspections were

needed in Unit 2 to further confirm the licensee's evaluation;

however, the inspector's had no immediate concerns regarding the

operability of the equipment associated with the splice / cable

assemblies in question. The licensee was informed that in

accordance with the guidance in Generic Letter 85-15, enforcement

action may be taken, in that the licensee should have clearly known

that these cable / splice assemblies were unqualified past the

November 30, 1985 EQ deadline. Pending further review this is a

Potentially Enforceable Unresolved Item. (50-295/86030-01(DRS);

b. Licensee Corrective Action

The licensee has committed to replacing all non-EQ splices and

non-EQ PVC cable used in the Unit 1 cubicle fan motors with

qualified EQ splice / cable assemblies prior to start-up, which

is scheduled for February 15, 1987. The Unit 2 splice / cable

assemblies will be replaced during the upcoming scheduled

maintenance outage in March 1987. Pending NRC review of licensee

corrective action this is an Open Item (50-295/86030-02(DRS);

50-304/86030-02(DRS)).

3. Potentially Enforceable / Unresolved Item

An unresolved item is a matter about which more information is required

in order to ascertain whether it is an acceptable item, an open item, a

deviation, or a violation. Potentially Enforceable / Unresolved Items are

unresolved items, which if ascertained to be a violation will be followed

up with enforcement action in accordance with NRC enforcement guidance on

environmental qualification. A Potentially Enforceable Unresolved Item

is discussed in Paragraph 2a.

4. Open Items

Open Items are matters which have been discussed with the licensee, which

will be reviewed further by the inspector, and which involve some action

on the part of the NRC or licensee or both. An Open Item disclosed

during this inspection is discussed in Paragraph 2b.

5. Exit Interview

The Region III inspectors met with the licensee representatives (denoted

under Paragraph 1) during an interim exit on December 31, 1986, and

discussed findings by telephone at the conclusion of the inspection on

January 14, 1987. The inspectors summarized the purpose and findings

of the inspection and the licensee acknowledged this information. The

licensee did not indentify any documents / processes reviewed during the

inspection as proprietary.
