As corrective action, the Operations staff was made aware of the need for an immediate review and correction of the noted discrepancies. A follow-up inspection was done by QC personnel on October 24, 1974. Discrepancies as noted in the comments of the principal inspector had been corrected at that time. We have reviewed your letter dated November 5,1974, and the following is our response to the apparent violation cited in that report. This response (G) v' details the violation, corrective action and the results achieved, steps taken to avoid future violations, and the date when compliance was achieved or will be achieved.

During the routine inspection of October 1-4, 1974, the principal inspector reviewed the Bypass and Jumper Log. Apparent inconsistencies in the maintenance of the log and its governing procedure, 1005.04, " Control of Bypasses and Jumpers", were noted.

As corrective action, the Operations staff was made aware of the need for an immediate review and correction of the noted discrepancies. A follow-up inspection was done by QC personnel on October 24, 1974. Discrepancies as noted in the comments of the principal inspector had been corrected at that time. No further discrepancies were evident.

To prevent future discrepancies, the plant operations staff has been informed of the need for a continued effort to maintain this log as required by the procedure.
