RADIOACTIVE GASEOUS EFFLUENT MONITOR Supplemental Report
~Back round:
Vent stack 5A wide Range Gas Monitor (WRGM), glWV-3547-1, was declared inoperable under Technical Specification 3.3.3.11, Radioactive Gaseous Effluent Monitoring Instrumentation, on April 12, 1988. Efforts to return the monitor to service within 7 days were unsuccessful due to problems with the low range sample flow transmitter calibration, and the event was reported in a Special Report on May 2, 1988 (Attachment 1). This supplemental report provides additional information about the flow transmitter problem.
Su lemental Information.'t was initially thought that the sample flow transmitters were defective since they could not meet the specified tolerance in the calibration procedure (MST-I0377). After repeated attempts to calibrate several spare flow transmitters were unsuccessful, a vendor representative was contacted. An engineering representative from Sorrento Electronics, a subsidiary of General Atomics, manufacturer of the 'radiation monitoring system, analyzed the sample flow problem and reviewed plant calibration procedures. The Vendor representative recommended changes to the calibration procedure.
The plant calibration procedure was revised incorporating changes recommended by the Vendor, and the sample flow transmitter .was successfully calibrated. The WRGM was restored to operation on August 20, 1988 at 2230.
