This Special Report concerns the operability of a radiation monitor.

ACCIDENT RADIATION MONITOR SPECIAL REPORT Descri tion of Event On May 28,
- 1991, at 16:37, the Waste Processing Building (WPB)
Stack 5
Exhaust System Radiation
- Monitor, RM-*1WV-3546-1, was declared inoperable when the monitor was removed from service to perform test procedure MST-I0375, WPB Stack 5 Accident Monitor RM->1WV-3546-1 Channel Calibration.
A failure identified during performance of this test procedure resulted in the monitor not being returned to service within 7
days.
This monitor is identified in Technical Specification Table 3-3.10 as Item 24(a).
The compensatory actions required by Technical Specifications to allow continued effluent releases are'(1) noble gas grab sampling every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and (2) continuous particulate and iodine sampling with auxiliary sampling equipment.
These compensatory actions were performed while the monitor was inoperable.
The monitor was out of service for greater than 7
days; thus a
Special Report is required to be submitted per the Technical-Specifications.
Cause/Corrective Action On May 28, 1991, radiation monitor RM-*1WV-3546-1 was taken out of service to perform MST-I0375.
During performance of this surveillance
- test, a
failed remote grab sample timer was identified."
The timer failure was due to failed capacitor which is now obsolete.
An alternate capacitor was identified and evaluated by the CP&L Nuclear Engineering Department as an acceptable replacement.
The failed capacitor is currently being replaced to correct the timer problem.
Following completion of
This radiation monitor should be operable by June 21,
- 1991, in the meantime compensatory actions will be continued.
