ACCIDENT RADIATION MONITOR SPECIAI REPORT Descri tion of Event On December 20, 1989 at 07:47, the Plant Vent High Range Noble Gas Monitor ARM-21AV-3509-1SA was declared inoperable when the monitor was found to be out of calibration during performance of test procedure MST-I0376, Plant Vent Stack Accident Monitor RM-21AV-3509-1SA Calibration. This monitor is identified in the Harris Technical Specification Table 3.3.10 as Item 18. The compensatory actions required by Technical Specification 3.3.3.11 to allow continued effluent releases are'(1) grab sampling every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and (2) continuous sampling with auxiliary sampling equipment. These compensatory actions were performed while the monitor was inoperable. The monitor was out of service for greater than 7 days, thus, a Special Report is required to be submitted per the Technical Specifications.

Cause/Corrective Action On December 19, 1989, Monitor ARM-21AV-3509"1SA was taken out of service to perform MST-I0376. The monitor failed the MST on December 20, 1989, due to the low range detector being out of calibration. On December 21, 1989, radiation control personnel completed a primary calibration on the monitor. On December 26 1989, while preparing to reperform MST-I0376, a separate problem was identified with the low range detector flow being too low. On December 28, 1989, the low range flow pump was replaced to correct the problem. RM-21AV-3509-1SA has been successfully tested per MST-I0376 and was declared operable on December 29, 1989.
