SEISMIC MONITOR SPECIAL REPORT BACKGROUND INFORMATION:

On September 4, 1989, at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> the Seismic Monitor System was declared inoperable due to the Seismic Annunciator Panel in'he Main Control Room having annunciators lit for no apparent reason.

Efforts to return the monitor to operable status within 30 days have been unsuccessful.

Thus, this special report is being submitted pursuant to Technical Specification 3.3.3.3 action a,

which requires the seismic monitor instrumentation to be operable at all times.

DESCRIPTION OF EVENT:

There are three floor mounted Triaxial Response Spectrum Recorders located on the 221 elevation of the Containment building which feed a

Response

Spectrum Annunciator that displays.

in the Main Control Room.

One of the thre'e recorders SR-DEISM-5204C is giving an indication on the Annunciator Panel in the Main Control Room for no apparent reason and was declared inoperable at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on September 4,

1989.

CAUSE:

The problem was discussed with the vendor and it is thought to be a defective Recorder Logic PC board.

This does not affect the passive portion of the recorder which was visually inspected on September 13, 1989, with no problems being identified.

The exact cause of the false indication on the annunciator panel has not yet been determined.

CORRECTIVE ACTIONS:

The vendor, ENGDAHL ENTERPRISES, is scheduled to be on site for an eighteen month calibration on October 30,

1989, and the required repairs will be made at that time.
