SEISMIC MONITOR SPECIAL REPORT BACKGROUND INFORMATION CAUSE:

On July 6, 1991, at 0100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br /> the Seismic Monitoring System was declared inoperable due to unsati,sfactory surveillance test results.

Operations Surveillance Test (OST-1042) is performed monthly to verify system operability by checking response to alarm and test signals. During the pre-test alignment for the test conducted on 7/6/91, it was noted that the event indicator on the Strong Motion Accelerograph (SMA-3) was not functioning properly. The test was deemed unsatisfactory and the monitor was declared inoperable.

Subsequent investigation performed by Technical Support and Maintenance personnel revealed that the indicator was operating properly and that the Seismic Trigger Test Switch must have been inadvertently depressed (bumped), causing the indicator to display white instead of black. After resetting the indicator to its correct position, OST-1042 was re-performed on 8/1/91. During this test, discrepancies were noted in the monitors playback strip recorder tape and the test was again deemed unsatisfactory. To provide accurate analysis of the strip chart tapes, copies were telecopied to the Seismic Monitoring System vendor, Kinemetrics Systems. Their conclusions were that the system was functioning properly and was capable of recording an event. The "play-back" function was the only part affected and this was due to the age of the cassette, the strength of the SMP-1 battery, the condition of the strip chart paper (Paper at end of the roll), personnel error (improperly loading of the cassette), or some combination of the above.

CORRECTIVE ACTIONS:

Maintenance Calibration Procedure (MST-I0240) and Operations Surveillance Test (OST-1042) were performed satisfactorily on 8/6/91 and the monitor was restored to operable status at 2205 hours0.0255 days <br />0.613 hours <br />0.00365 weeks <br />8.390025e-4 months <br />.
