Vent stack 5A wide range gas monitor, (WRGM) 01WV-3547-1 was declared inoperable under Technical Specification 3.3.3.11, Radioactive Gaseous Effluent Monitoring Instrumentation, on April 12, 1988, at 0830. This was due to the loss of sample flow that would result during calibration of the sample flow meter.
Efforts to return the monitor to operable status within 7 days have been unsuccessful, thus the need for this special report, which is submitted pursuant to Technical Specification 3.3.3.6, Action C.
Descri tion of Event:
On April 12, 1988, the sample flow meter was scheduled to undergo a routine calibration. At 0830 the equipment was declared inoperable and the calibration (MST-I0377) was started.
Iamediately upon starting, it was discovered that the procedure was in error, and a Temporary Change was needed to correct the procedure. This process was completed and the calibration was restarted at 0848 on April 13, 1988. At 1100 the calibration was stopped again due to an incorrect data table in the procedure.

Radioactive Gaseous Effluent Monitor S'pecial Report

Vent stack 5A wide range gas monitor, (WRGM) 01WV-3547-1 was declared inoperable under -= Technical Specification- 3.3.3.11, Radioactive 'Gaseou's'ffluent Monitoring Instrumentation, on April '12, 1988, at 0830. Thxs was due to the loss'f sample flow that would result during calibration of. the sample flow meter.
Efforts to return the monitor to operable status within 7 days have been unsuccessful, thus .the need, for this special= report, which is submitted pursuant to Technical Specification 3.3.3.6, Action C.
Descry tion of Event:
On April 12, 1988, the sample flow meter .was scheduled to undergo a routine calibration. At 0830 the equipment: was declared inoperable and the calibration (MST-I0377) was started.
Immediately upon starting, it was discovered that the procedure was in error, and a Temporary Change was needed to correct the procedure. This process was completed and the calibration was restarted. at 0848 on April 13, 1988. At 1100 the calibration was stopped again due to an incorrect- data table in the procedure.
This required an Advance Change to be made to the procedure and it was not approved until April 14, 1988. The procedure was restarted at 1230 on April 14, 1988, and continued until it was found that the Low Range flow transmitter could not be -
calibrated. A work request was initiated to replace the defective transmitter, and a subsequent check of spare parts revealed that a replacement part of the same type was not available. A transmitter of a different type was located but was not approved for use, so a replacement transmitter has been ordered.
Calibration continued until April 19,. 1988, when it was complete with the .,exception of the Low Range flow transmitter calibration and final closeout checks.
The compensatory actions requ'ired by Technical Specifications 3.3.3.11 Table 3.3-13, Actions 46, 47, and 49 required an auxiliary sampler to be hooked up to continuously monitor the stack- while the installed effluent monitor was inoperable. This is being performed and 'all compensatory measures 'for gaseous effluent and accident monitoring for stack 5A were satisfied.

Cause.
The monitor wa's declared inoperable initially due to the loss of sample flow that would occur while attempting calibration of the sample flow transmitter. The primary cause of the delay in restoring the monitor to service is the unexpected problems in procuring the correct type of flow transmitter. Two transmitters were waceivM buxom Aux', Zns~enks, 2nc. ~ ~xil 22, Nut both were found to be defective and had to be returned to the vendor.
The cause of the failure of the Low Range flow transmitter, which was discovered during the calibration of the sample flow meter, is not known.
Corrective Action:
The corrective actions taken to restore the monitor to operable status were to order a new Low Range flow transmitter of the correct type and complete the calibration procedure. The monitor will be returned to service as soon as a workable transmitter is received from Kurz Instruments. A scheduled corn'pletion date available at this time but will be provided later in a is'ot supplemental report.
