USNRC 10CFR Part 21 Notification This is to notify the US Nuclear Regulatory Commission, in accordance with the provisions of 10CFR -Part 21 of a deviation identified by Flowserve Corporation.
On May 13, 2008, Flowserve Inc. Raleigh N.C. was notified by SCE&G -VC Summer Nuclear that a pair of ASME Section III Class 3, NPS 16" Classl50 Butterfly Valves with Fail Open Air Actuators, installed in the site service water system (SW) in May 2008 during RF -17, failed to open during post installation testing against design differential pressure.
The plant was in a safe shutdown condition at the time of the testing.The valves are used for isolation and are normally closed but fail open during loss of power.The valves must also open upon start of the SW booster pump to provide SW cooling to the RBCU to maintain Reactor Building temperature.
A number of issues were discovered during installation and subsequent evaluation of these valves and were corrected by SCE&G under direction of Flowserve Field Service Technicians and Flowserve Engineering support. The as installed configuration of the valves and actuators included site modifications and parts supplied by Flowserve to decrease the valve operating torque and increase the actuator opening output torque. After the modifications, the valves were tested and opened at full design differential pressure and closed as required.The root cause of the failure has yet to be determined.
This failure is under investigation by Flowserve Engineering.
An examination of the valve hardware will be necessary to complete the investigation and failure analysis.
The investigation will proceed after replacement of the valves/parts and the release of the original valves/parts by SCE&G to Flowserve.

The SCE&G specifications required equipment unique to the application.
Therefore, this incident does not affect Flowserve, Raleigh equipment provided to other nuclear facilities.
