SUBJECT:

SHUTDOWN FOR GREATER THAN 48 HOURS On January 8,1988, with the plant operating at 100% power, a reactor trip / turbine trip occurred at 3:03 pm (PST), on overpower delta temperature (OP Delta T). The coincidence of the performance of a channel functional test on protection set IV and a component failure in protection set I established the 2 out of 4 logic for OP Delta T reaci.or trip. Systems functioned as expected, including feedwater isolation and auxiliary feedwater system actuation, with the exception of the N32 source range channel which was energized but failed to indicate. The cause was determined to be a dirty contact in the cable connection to the pre-amplifier. While in Mode 3 af ter the reactor trip, indications of containment leakage led to discovery of leakage at the secondary manways on steam generators B and D (total of 4 manways). Following an engineering evaluation and concurrence from the NSSS vendor, the lcakage was stopped on the 'D' manways by increasing the torque on the bolts of the manway covers. The use of sealing compound is planned on the 'B' manways because the torquing was not fully effective. Plant restart is expected in early afternoon January 12, 1988, following completion of the Furmanite repair.
