The information presented is as initially received without verification or evaluation and is basically all that is known by Region V staff on this date.

FATIGUE FRACTURE ON SAFETY INJECTION FILL LINE ON "A" ACCUMULATOR On May 12, 1987, at 10:48 am, during a refueling outage in Mode 6, a fatigue fracture occurred on a 1" safety injection fill line on the "A" accumulator tank which had a 360 d: gree circumference break on the field weld and 2000 gallons of 2000 ppm borated water drained on to the containment floor. The accumulator was filled and presst.rized at 585 psig. The field weld was the first weld from the tank. No personnel were injured and there tas no release of activity. The licensee has visually examined remaining lines and welds and plans to dye penetrant (d.p.) test them.

As of May 14, 1987 a.m., the licensee found two additional accumulator lines with cracks via the d.p. test. The licensee's plans are to grind out and u.t. the defects. The licensee intends to perform a thorough root cause analysis of these weld problems, and will develop appropriate corrective actions to prevent further occurrence. Region V will follow-up on tha licensee's analysis program.

The licensee issued a press release.
