Re: Activity found in the Normally clean Waste Treatment (WT) Por/J Normal weekly sampling for Feb.19,1997of the WT Pond by Environmental Chemistry identified a single isotope of Tc-99M with a concentration of 1.075E-7 pCi /ml.

The Waste Treatment (WT) Pond receives sanitary waste effluent from the McGuire Site rest rooms.

After treatment, the WT Pond empties to the Waste Water Collection Basin (WWCB), which discharges to the Catawba River. These effluent ponds are normally expected to be free of activity therefore no Radiation Monitor is associated with this system.

Follow-up investigation identified an employee, which had received a medical injection of 36.8 mci of Tc-99M on 2/12/97. Subsequent pond samples identified no activity.

The source of the WT Pond activity is attributed to a radio. pharmaceutical treatment input to the WT Pond through the sanitary waste system.

Shift Support Supervisor Radiation Protection McGuire Nuclear Station RE: Discharge Canal Minimum Flow Interlock inoperability exceeded the 30 day SLC (16.11 2) action. PIP

  1. M-97-1948, PIP #M-97-2093.

On 4/19/97 the Discharge Canal Minimum Flow Intertock (DCMFI) secured a WMT release inappropriately. It was declared inoperable and a work request generated.

On 5/7/97 an l&E work crew performed a normal ACOT PM as part of their trouble shooting plan. No problem was found with the function as tested by the PM procedure. RP reviewed the procedure and found the test procedure to be inadequate to validate operability of the DCMFI in relation to the failure that was observed. Therefore the component was left in an inoperable status. Inoperable actions were being taken as required by SLC. At no time j during this inoperable period have ics than the minimum required RC Pumps operating for dilution been '

observed.

PIP M-971948 was generated on 5/7/9/ to evaluate the test method for resolution. The PIP had a discovered i date of 4/19/97 and due date of 5/19/97. The PIP was incorrectly assigned to *he Maintenance organization I instead of Engineering, so the first knowledge engineering had of this 4/19/97 event was on 5/15/97 at 3pm. The procedure change needed was fairly involved. The process did not occur quickly enough to avoid exceeding the 30 day SLC limit. PIP M 97-2093 was generated to evaluate this process failure.

l The following resolutions have been implemented: 1) PT/0/B/4500/004 was revised by engineering on 5/20/97 and placed in the file by the procedure group on 5/22/97. Changes made provided a realistic plant configuration to more thoroughly check the Programmable Logic Controller (PLC). The input and output modules for this PLC j have status lights and the newly revised procedure takes advantage of this information. This change will ensure '

that an input will not be left in a different state than its 'as found'. 2) lAE maintenanco ran the revised PT per corrective work order 97041772 on 5/30/97 and had no difficulties. No problems with the PLC logic were detected and the expected results were obtained. The DCMFI was declared operable on 6/4/97 at 18:30 hrs after acceptance by engineering. 3) PIP M-97-1948 has been evaluated. The apparent cause was that the way the procedure has been performed in the past has caused the sliding links in the ATC cabinet to wear out. This most likely caused an incorrect PLC status (i.e. RC pump was running and a link was inadvertently left open so the PLC ,

kxjc read it as not running). This would explain why the release was terminated when it should not have been. l The inappropriate termination on 4/19/97 was conservative. 4) The PIP corrective action includes a work request for replacement of the wom out sliding links in 1 ATC13 and 2ATC13.

McGuire Nuclear Station inoperable Instruments Exceeding Selected Licensee Commitments (SLC) Limits -

2 EMF 31 was discovered to have a failed detector during a calibration performed on March 16,1998. It has-

. been determined that the EMF detector failed on August i1,1997 and therefore exceeded the 30 day inoperable limit. It was returned to service on March 17,1998. An investigation is in progress and will documented under PIP 2-M-98-0940.

No other monitoring instrumentation exceeded the 30 day limit in 1997.
