McGuire Nuclear Station i TBS misalignment to the WWCB Resulting in an Unplanned Release

PIP 0-M96-0989 On April 10,1996, Operations and Chemistry were making an alignment for the unwatering of the unit-2 condenser. To allow for unwatering (WU) to the Waste Water Collection Basin (WWCB),

unit-2 Turbine Building Sump (TBS) was aligned to unit-1 TBS. Both sumps were then to be aligned to the WC Discharge. Due to a miscommunication, the TBS's were aligned to the WWCB discharge pathway for a period of one hour. This resulted in an unplanned release.

The unit-1 TBS had been sampled on 4/10/96 at 0300 and had 5.0.5E-06 C1/mi tritium. The a unit-2 TBS had been sampled on 4/10/96 at 0305 and had 4.78E-06 Cl/ml tritium. It has been conservatively estimated that the unit-1 TBS pumped out to the WWCB two times during the one hour misalignment. (This allows each sump to have filled completely one time). Based upon measurements taken by engineering, the volume of a normal TBS pump down is 1600 gallons for a total release of 3200 gallons. The total Curies released is estimated as 5.95E-5 Cl of tritium calculated as follows

McGuire Nuclear Station Unmonitored Release From Waste Handling Area (WHA) Ventilation

! On June 21,1996 at 1041 hrs., the motor control center providing power to EMF 53 (WHA

ventilation exhaust radiation monitor) and the attemate sample pump was de-energized for PM
work. For approximately 48 minutes, the ventilation exhaust was running without being 1- monitored.

4 Collection of the continuous particulate and iodine samples for the WHA is performed on a l weekly basis, along with a gas grab and a tritium sample. A review of all samples from the WHA l 4

for the month of June showed no activity was detected. This is generally a clean area. There is 1

no reason to suspect that any activity was released during those 46 minutes.

McGuire Nuclear Station Unplanned, Unmonitored Release to RC

Reference:

PIP 0-M96-2319 On August 16,1996, an alignment was made to pump the volume of water from Waste Monitor l Tank "B" to Waste Monitor Tank "A" for the purpose of performing a functional verification on

flow loop OWMLP5130. An R&R was prepared specifically for this alignment; the discharge 3

piping was double isolated. Part of the functional was to receive a loss of flow signal on EMF 49L Even though the double isolation of the discharge piping should have prevented flow through the EMF, it was found necessary to close the intet valve to the EMF in addition to the original R&R. After successful completion of the functional, the water was pumped back into

Waste Monitor Tank "B", but the resultant volume was 5% less than the original volume. There l were only two pathways the water could have taken, one to the YM header, the other to the RC discharge. The YM header was sampled and no activity was found. Therefore, it is assumed the water went to the discharge header. The volume, based on the 5% loss, is estimated to be 175 Gallons.

, During the time of this alignment, the Unit-1 Containment Ventilation Unit Condensate Drain Tank was in continuous release mode, so WP35 (final isolation valve in the discharge piping) was in the open position. Since there was no control past the double isolation of the above .

mentioned R&R, the water that entered the discharge header is considered to be an unplanned

, release. This is a conservative decision because it is estimated that it would take 150 gallons of water to reach the discharge point.

, Also, during the work on OWMLP5130, EMF 49L was declared inoperable until the functional i could verify the bleed / vent of the lines was successful. The readings on EMF 49L were the same during the functional (-5100 cpm) as they were during the next release (LWR 367) which was the same water that had been transferred between the tanks. The functional itself proved the EMF to be operating properly. However, since the EMF was declared inoperable at the time, this is considered to be an unmonitored release.

The total beta-gamma Curie contribution from the 175 gallons of water was 2.69E-6 Curies.

These Curies were accounted for in LWR 367 by adding the 5% lost volume to that release.

On September 26,1996, the CM (Main Condensate) system backed up into the YM (Demineralized Water) system. This caused the YM storage tank to overflow to the roof drains, t

which are routed to the Waste Water Collection Basin (WWCB). A sample of the YM tank overflow revealed 1.7E-4 pCi/mi of tritium activity. The result was an unplanned, unmonitored release to the Catawba River via the WWCB.

The total gallons released during this event was conservatively estimated to be 8550. The resultant Curies released was calculated as follows:

1.7E - 4 Ci / ml x 8550 gals.x3785ml / gal x IE - 6Ci / Ci = 5.50E - 3Ci

- These Curies are accounted for through ERL #11.

PIP 0-M-96-2855 It was discovered on October 7,1996 that tritium activity had been detected in some of the WT Pond weekly samples collected by the Environmental Chemistry group. The WT Pond empties to the Waste Water Collection Basin (WWCB), which discharges to the Catawba River. Since this system is not expected to have activity, there is no radiation monitor associated with WT. Therefore the tritium activity resulted in an unplanned, unmonitored release.

There were a total of seven samples that had positive activity. They are listed in the table below with the WT effluent volume for the representative week.
