The name and organizational affiliation of the team menber with badge number, Social Security Number, and Level of Clearance should be provided to the Diablo Canyon Plant Manager in an enclosure and should be treated as Official Use Only (0U0) information. The enclosure should not be placed in the Public Document Room.
