Dear Dr. Grace:

VIRGINIA ELECTRIC AND POWER COMPANY NORTH ANNA POWER STATION UNITS 1 AND 2 NEW SERVICE WATER SPRAY ARRAY SUPPORT SYSTEM 10CFR21 REPORT on November 7, 1986, Virginia Electric and Power Company notified NRC Region II of a potential defect involving a substantial safety hazard at North Anna Power Station Unit Nos. I and 2.

In accordance with the reporting requirements of 10CFR21, the following information is submitted:

A. Name and address of individual informing the Commission:

C. Identification of the firm supplying the basic component which fails to comply or contains a defect:

Fabricator: Rockwell Engineering 13500 South Western Avenue Blue Island, Illinois 60406 D. Nature of the defect or failure to comp 1/ and the safety hazard which is created or could be created by such defect or failure to comply:

On Septenber 15, 1986 a routine post veld QC inspection initially revealed laminations in the web of a steel column. Rockwell Engineering Co., the seller, was notified along with Inland Steel, Rockwell's subsupplier. Inland stated that the defect is known as " pipe type lamination" or " ripe" which is rarely encountered in finished products.

Inland reviewed the production history of the defective steel (Inland heat number 17802) and informed Virginia Electric and Power Company that an ingot poured from heat 17802 could develop " pipe" due to the manufacturing conditions that existed at the time. At that time Inland believed that the defect was an isolated case.

On October 23, 1986, subsequent ultrasonic testing (UT) of steel from the suspect Inland heat number revealed several additional defective columns. Rockwell Engineering was notified and a non-destructive engineering specialist from Inland Steel visited North Anna on October 28, 1986. The specialist confirmed the findings, verifying that the defect was not an isolated case.

Review of the Rockwell traceability documentation indicated only W10x49 wideflange members were supplied to Virginia Electric and Power Company from Inland heat 17802. W10x49 members were also supplied to the company from other Inland heat numbers as well. However, given the fact that production problems were experienced during Inland heat 17802 coupled with the high level of reliability normally experienced with hot rolled structural steel, the scope of concern over the lamination defects is limited to the material being used from Inland heat 17802.

Positive identification and ultrasonic testing of suspect materials is to be completed by December 1, 1986. At this time, a repair / replacement plan will be developed. A Non-Conformance Report will be written upoa completion of the UT program to document resolution of defects.

Members containing defects as described above may not be suitable for use in the safety-related Service Water Spray Array Support System.

E. The date on which the information of such defect or failure to comply was obtained:

November 5, 1986 F. The number and location of all such components in use at, supplied for, or being supplied for one or more facilities or activities subject to the regulations in this part:

The number and location of the affected components at North Anna Power Station will be identified in the aforementioned Non-Conformance Report.

Virginia Electric and Power Company will perform ultrasonic testing of the suspect materials at North Anna Power Station. This testing will be completed by December 1, 1986. Once the extent of the problem is known, a repair / replacement plan will be developed.

Discussions are continuing with Rockwell Engineering and Inland Steel with regard to problems with the structural materials being provided.

H. Any advice related to the defect or failure to comply about the facility, activity, or basic component that has been, is being, or will be given to purchasers or licensees.

The firms identified in paragraph C above have been notified concerning this problem.
