Part 21 Notification for Fairbanks Morse Engine Division Model 38TD8-1/8 Piston Pin (Symbol 16200274)
Dear Sir:
During wear in of two commercial power units at Bay City, Michigan, starting on October 21 and ending on December 4, 1989, there were several failures of piston pins / bushings on the upper pistons. These engines are 900 rpm turbocharged units. Subsequently, a thorough investigation into the cause of failure resulted in finding that the root cause was the surface pattern on the piston pins. Our piston pin supplier had changed the honing process on the piston pins, creating a crosshatched finish that was within surface finish specifications, but did not provide the correct surface-pattern.
Under the right conditions, this crosshatched pattern can cause the pins to stop rotating, resulting in bushing failure. This condition only occurs on upper piston assemblies in 900 rpm turbocharged 0.P. engines. The lower piston assemblies are not affected by this condition.
We have furnished pins from August 1986 to present that are suspect. A list of sites to whom we have sold spare / replacement parts is enclosed. They are being notified-of the potential problem by copy of this letter.
Fairbanks Morse recommends the following actions be taken by the utilities:
- 1. Remove from stock piston pins furnished on the referenced purchase orders (enclosed) and return them to F.M. for inspection and replacement as necessary. Contact Ted Stevenson, phone 608/364-8424, to arrange for return and replacement.
- 2. Should any of .the pins furnished have been installed in an upper piston assembly, these pins should be replaced at your earliest opportunity.- Contact F.M. Product Service's Tom Stull, phone 608/364-8161, to arrange for inspection at the site. We will inspect the pins and provide replacements as necessary.
