Under the provisions of 10 CFR 21, NRC Region II was notified on September 13, 1985 L that a potential significant deficiency or substantial ~ safety hazard

' existed at North Anna Power Station Unit Nos. I and 2.

In accordance with the reporting requirements of the above regulations, the following information is submitted:

On September 9, 1985 at 0139 security-personnel discovered that a station employee with restricted clearance had unauthorized access to certain station vital areas (as defined by the North Anna Security Plan). The restricted clearance badge, number 1329, was issued to the employee, an operator trainee, on August 2, 1985 along with a corresponding electronically encoded key card. A restricted clearance authorization allows access into the protected area only via the key card. On September 9, 1985 the key card issued to this employee was discovered to be incorrectly encoded as it was read by the electronic access control system as badge number 1201. The key card was tested in other card readers with the same results. Badge number 1201 is a currently issued badge with unrestricted clearance and authorization for access to several station vital areas. Consequently, the employee could have gained undetected access to these areas via the defective key card; however, research indicated no unauthorized entries were made.

The defective key card, model #1310, was manufactured and encoded by Pyrotronics. In order to prevent recurrence, currently issued key cards were checked using the electronic access control system to verify that they were properly encoded. This was completed on September 10, 1985 and yielded another key card which was improperly encoded. -This second defective key card would not 'have allowed access to any vital areas.

Additionally, new unissued key cards on site were checked to ensure no other defective key cards existed. This was completed on September 11,

~ 1985 and resulted in the discovery of three additional key cards which l

were improperly encoded. The five defective key cards were subsequently destroyed. This event was determined to be a substantial safety hazard as described in NUREG 0302, Revision 1 and therefore reportable under 10 CFR Part 21.

As noted above, on-site key cards have been tested and the five found to be defective were removed from service and destroyed. The North Anna security department will test new cards upon their arrival in order to prevent recurrence.
