Interim Peport - Potential Nonarnpliance Presented in Referenced Documents Gentlemen:

The following information supplements previous submittals and gives the current status of our continued investigation.

Southwest Fabricating & Welding Co. has reviewed all contracts listed in our Nuclear Iog Book to determine where Class 2 nozzles were furnished that required radiography and reinforcing pads.

In addition to those previously reported at the Clinton, Byron and Braidwood sites, our records show that 900 Branch Connections in this category have been furnished to the North Anna 1 and 2 jobsites.

Itms supplied to North Anna were under Stone & Webster Engineering Corporation P. O. NA-64 and NA-1064 for North Anna Unit #1 and #2, respectively. Mr. G. A. Helm, Manager of Quality Assurance for the Virginia Electric Power Co., was notified on Septaber 30, 1983 and asked to review radiographs of the subject welds (see Attachment "A") .

North Anna Units #1 and #2 were fabricated in accordance with USAS R31.7, 1969 Edition.

Our review of the Cincinnati Gas and Electric contract for the Zinmer pmject, revealed that no nozzles in this cateoory were furnished.

However, four of our shop detail sheets for this contract are missing.

Eight pipe sgols were fabricated frcm these shop detail sheets.

Nuclear Regulatory Cmmission October 4, 1983 Page Two Mr. John Wald, Director of Operations QA for Cincinnati Gas and Elec-tric Co., was notificd on September 30, 1983 and asked to review drawings for these pieces and report results to Southwest Fabricating

& Welding 03. (see Attachment "B") . This unit was fabricated in accordance with ASBE Section III,1971 Edition.

The review of radiographs at the Byron and Braidwood sites is now canplete. Mr. Paul Donovan of Canmonwealth Edison CaTpany inforned the writer on September 29, 1983 that all suspect welds (88) had been reviewed and accepted as meeting code requirements. Mr. Donovan stated that Camenwealth Edison will subnit a letter to the Nuclear Regulatory Caumission Region III stating that they have no reason to submit a 50.55(e) report or take any further actia on this matter.

Based on a Telcon with Mr. Mike Pacy of Illinois Power Company on October 3,1983, they are continuing the investigaticn at the Clinton jobsite. Illinois Power Canpany has engaged the services of Mr.

01uck Hellier to perfonn the investigation. Southwest Fabricating will keep you psted as this investigation progresses and results are made known.

m de by Southwest Fabricating & Welding Co., I detected one incident , d i.f i :n where we possibly had not canplied with ASME III requirenents for i radiography. This circumstance occurred on an ASbE III Class 2 .N. ;p pipe spool furnished in accordance with the 1974 Edition, Su:rmer

1974 Addenda. A nozzle was welded into the header pipe at a 450 angle and the attaching weld was subsequently covered with a rein-forcing pad. No deficiencies have been found in nozzles installed at angles other than 450 '

Code requires, in this circumstance, that the nozzle to header

- attachment weld be radiographed prior to installing the reinforcing pad. Radicgraphs of the weld that caused initial concern indicated that the weld surface had not been suitably conditioned prior to radiography. Surface irregularities existed that could mask defects.

Cbviously, after the reinforcing pad is installed, the condition is '

no longer accessible for visual inspection and, therefore, causes weld quality to be indeterminate as represented on film. bbny welds having the sane configuration have been reviewed and found to be in ccrnpliance with Code requirarents.

Southwest submitted this deficiency to Nuclear Regulatory Camussion Region IV as a potential violation of Code and, in canpliance with 10 CFR Part 21, we have investigated all nuclear contracts to deter-mine the location of all welds installed in this manner and requiring 1 radiography.

During a recent review of radiographs taken on nuclear piping welds made by Southwest Fabricating & Welding Co. , I detected one incident where we possibly had not ccinplied with ASME III Code requirsents for radiography. A nozzle was welded into the header pipe at a 450 angle and the attaching weld was subsequently covered with a rein-forcing pad. It is unlikely that such welds exist, since none were found on the renainder of the drawings.

Please advise the undersigned of the results of your review so that this issue can be closed with the IEC.
