Colt Industries-Fairbanks Morse Engine Division Model 38TD8-1/8 Emergency Generators Connecting Rod Nuts (Bolts)
Part 21 Report Gentlemen:
During Colts factory assembly of connecting rod and rod bearing caps on a Model 38TD8-1/8 engine it was observed that the fit up of the nut to the mating surface of the connecting rod cap was not parallel. Further inspection led to the determination on November 25, 1985 that some nuts in stock have faces that are not perpendicular to the thread pitch line within the specified tolerance. This can result in a bending load being imposed on the bolt, inducing stresses beyond the design limit.
The suspect nuts are from vendor shipments received from April 9, 1984 thru the present. Nuts produced before this date were manufactured by Colt within the specified tolerance.
Utilities that could have defective parts are:
1)
Baltimore Gas & Electric Co.; Calvert Cliffs, On Site Spare Engine only.
2)
Virginia Electric & Power Co.; North Anna, On Site Spares.
3)
Philadelphia Electric Co.; Limerick Station, On Site Spares.
4)
Detroit Edison Co.; Fermi #2, On Site Spares.

,o U.S. Nuclear R:gulatory Commission November 26, 1985 Our course of action on this potential defect is to:
A)
Replace all nuts and bolts in the assembled Calvert Cliffs On Site Spare Engine.
B)
Eetermine which nuts (bolts) shipped as on site spares have been assembled (Torqued Up) and replace both nuts and bolts.
C)
Determine which nuts have not been assembled (Torqued Up) and replace these nuts.
A copy of this report is being forwarded te the Utilities involved.
We will keep you informed of our progress in this matter.
