1) Flow rate determining devices = +13%
2) Counting error = +15%
3) Sample preparation error = + 3%

4 ATTACHMENT 2 INOPERABLE INSTRUMENTATION

Attachment Inoperable Instruments (Period Covered July 1986 through December 1986)

11. Containment Ventilation Unit Condensate Drain Tank Continuous Composite Samplers (Units 1 and 2).

On June 1, 1984, Units 1 and 2 CVUCDT Composite Samplers were declared inoperable. The samplers have remained inoperable since that date and have exceeded the' thirty (30) day inoperability limit as stated in Technical Specification 3.3.3.8, Table 3.3-12, Action 33.

New' Samplers have been installed and are presently undergoing testing by I&E and projects personnel.

2. Unit 1 Containment Ventilation Unit Condensate Drain Tank Flow Rate Monitor.

On 10-12-86 the CVUCDT flow rate monitor was declared inoperable.

Inoperability is due to the replacement of the Flow Element (1WLFE-5900) and Flow Transmitter (1WLFT-5900). This replacement is being performed in conjunction with the installation and calibration of the new CVUCDT composite samplers, (see item number 1). Calibration is scheduled for January 1987, pending arrival on site of required calibration equipment.

Pump performance curves are presently being used to estimate flow. This period of inoperability exceeds the thirty (30) day limit as stated in Technical Specification 3.3.3.8, Table 3.3-12, Action 34.

3. Radwaste Facility Ventilation System Flow Rate Monitor.

On 11-04-86 the RWF flow rate monitor was declared inoperable by McGuire I&E personnel. This period of inoperability exceeds the thirty (30) day limit as stated in Technical Specification 3.3.3.9, Table 3.3-12, Action 36.

During periodic maintenance by I&E the system flow transmitter (MVAFT 5300) was found to contain moisture, originating from the VI air supply line, the flow transmitter was replaced and calibrated. However, the transmitter could not be functionally tested due to the RWF ventilatior being secured. The RWF ventilation has been secured pending completion of a modification to install a particulate filtration system. Functional testing of the flow transmitter will be rescheduled upon completion of the stated modification.

Pursuant to McGuire Technical Specification 6.9.1.7, attached is the Subject Report for the period of July, 1986 to December 1986.

Attachment 2 contains a summary of effluent monitoring equipment which was out of cervice in excess of the time allowed by McGuire's Technical Specifications.

As a result of radiation releases from McGuire Nuclear Station, no member of the public received an annual calculated dose exceeding those set forth in 40CFR Part 190. This determination was made considering that there are no other fuel cycle facilities nearby which would significantly increase the dose to any member of the public.
