2.2 RESPONSIBILITY l
The Quality Control Supervisor shall be responsible for implementation of this instruction. ;
2.3 PERSONNEL QUALIFICATIONS All QC Inspectors (QCI) performing inspections in accordance with this instruction shall be trqined, qpalified and certified in )
accordance with the requirements of QI-QAP-2.1-5.
2.4 NON-CONFORMANCE CONTROL l During QC inspection, characteristics which are identified as !
unacceptable shall be documented in accordance with QA Procedure CP-QAP-16.0. QCI shall document the CDR number on the applicable l process documentation.
3.0 INSTRUCTION 3.1 COMPONENT SUPPORT CONFIGURATION CONTROL I 3.1.1 Compenent Support Drawings Component support construction drawings issued by Engineering shall be used to define structural configuration. Additionally..
" Typical Inspection Drawings" are issued by Engineering to further define fabrication / installation inspection details, generic component modification methods, and Engineering approved alternative fabrication / installation processes. Component support drawings and typical inspection drawings are issued by Engineering through the Site Document Control Center as
" Controlled Documents" in accordance with Section 7 of Reference 1-D.
Engineering ray issue DCA's (Desigt Change Authorization) to l document changes or deviations to specified design / construction requirements.
Prior to final QA acceptance of a component support. Engineering will issue a yendor Certified Drawing (VCD) f o r 21s " and larger pipe or a Design Reviewed Drawing (DRD) for 2" and smaller pipe.
This drawing vill incorporate all outstanding CMC /DCA.

If the VCD/DRD or Category II drawing does not agree with the Construction drawing and CMC /DCA, Paper Flow Group (PFG) will issue proper documentation for QC to re-inspect to )
verify compliance. In addition, the QC Document Review Group will assure all drawing design changes have been implemented, )
verified and documented whether identified by PFG or not. '
Implementation of the above will be verified per CP-QAP-12.1.

1 3.1.2 Component Support Fabrication / Installation Process ;
3.1.2.1 Process - Flow Component Support Fabrication / Installation Process Flow, (Attach-ment 2.) represents ~ the typical process flow from Engineering issuance of the drawing to final acceptance of the component support in accordance with CP-QAP-12.1.
1 3.1.2.2 Component Support Work Package (WP) Contents '
Package Flow Group (PFG,) upon receipt of the controlled con-struction drawing, will prepare the fabrication / installation WP. ,

When a support is voided, craft shall notify QC by means of a MWDC and QC shall document as such by signing the appropriate I hold points.
3.2 MATERIAL IDENTIFICATION 3.2.1 Material Identification Requirements 3.2.1.1 Vendor Supplied Component Supports and Bulk Items l i
Vendor supplied "NPT" stamped component s t.p po r t s' sfiall bear manufacturers serial number. . Component supports requiring field ;
welds at installation shall bear mechanically marked, unique l identification on each part traceable to the vendor data package.
NPSI supplied hardware is identified by miscellaneous markings such as NF, NR, etc; ITT supplied hardware is identified by alpha markings A, B, C, etc. NPSI & ITT bulk items shall not be interchanged for vendor supplied component supports, unless authorized by Engineering.
3.2.1.2 Component Standards Support (Catalog items)
Component standards supports, such as pipe clamps, U-bolts, etc.,
shall be traceable to the material type and grade until the material is received and verified by CC. Component standard supports shall be controlled until issuance for fabrication /
installation in accordance with Brown & Root Quality Procedure CP-QAP-8.1.

l The acceptability of a component standard support, or fastener, for fabrication / installation is identified by the vendor's unique marking

('i . e . , letter code, MIC no., serial no., etc..) or a Brown and Root
, applied color code (Class 1 - Black, Classes 2 and 3 - Green.) )
Dimensional verification through the use of " Typical Inspection Drawings" (i.e., FXB , ' FXN , etc.) or a specific material grade stamp (i.e., B7 for bolts, 2H for nuts, etc.) are acceptable identification means for fasteners utilized in Non-ASME applications.
3.2.1.3 Brown and Root Fabricated / Modified Component Supports Brown and Root fabricated Class 1. component supports shall bear unique .
markings on each item of structural steel used in the f abrication of

the component traceable to a Certified Material . Test Report (CMIR.)
Structural steel used in the fabrication of Class 2 and 3 component .
supports shall bear unique identification traceable to the material l type and grade. Materials used to modify vendor-supplied component i supports shall also comply with the preceding requirements..
3.2.1.4 Material Traceability Requirements Material for component supports shall carry identification markings 1 which will remain distinguishable until the fabrication and {
installation of the component support is accepted; identification l' marking shall be documented on the MIL /SAVC. If the original identification markings are cut off/ divided or covered by additional fabrication, the identification shall be accurately transferred to assure ident(fication of' each piece of material during subsequent fabrication or installation. QC shall verify marking transfer prior to separation / coverage and document on the MIL /SAVC.
l NOTE: When shim material is used to maintain pipe clearance and ;
traceability markings are covered by additional fabrication. I reverification of traceability after installation is not required by QCI.
For Seismic Category II , component supports, which were fabri-cated/ installed before incorporation under this program, field nCI shall assure that the configuration (size) of all material installed complies with the material in the Bill of Materials (BOM) of the drawing; and document acceptability on the MIL / Component Support Structural Inspection Report (CSSIR) accordingly.
3.2.2 Material Identification Documentation.
3.2.2.1 Material Identification Log (MIL)/ Structural Assembly Verification Card (SAVC)

NOTE: The SAVC is utilized to document material acceptability for Moment Restraints and Equipment Supports.
NOTE: The shop / field QCI shall compare entries on the MIL /SAVC with the respective MR to assure that the material has been verified by Receiving QCI and is acceptable for its intended use. The llR number shall*
be recorded on the MIL /SAVC. Copies of MRs for bulk material verified by' shop QCI are not required to be I included in the support package. I

3.2.2.2 Material Requisition (MR) )
The MR is used by Construction to requisition material for fabrication / installation.
Entries on the MR shall be compared to l the material being requisitioned and acceptable verification shall be denoted by the Receiving QCI signature on the MR.
3.2.3 Material Salvaging Salvaging of component support parts such as structural steel, snubbers, moment restraints, etc., shall be accomplished as follows:
I 3.2.3.1 Salvaging ,
When an item is salvaged for use on a support other than the one for which it was desig,nated, the original support mark and serial number; or original mark, MIC or heat number; or original mark and heat code shall be distinguishable on the item.
If vendor or B&R fabricated supports have been previously accept-
~ ed and the identification numbers are not distinguishable, the identification numbers may be determined by applicable installa-tien documentation used te originally accept the material. The material identification numbers shall be transferred to the items prior to separation.
If vendor fabricated compenent supports with ASME name plates are salvaged with welds and used on supports other than those for which they are designated, the name plate (s) shall remain attach-ed to the item. When all vendor velds are removed, the name plate (s) shall be removed, provided the original support number is transferred to the individual vendor parts that are to be salvaged.

Authorization to remove name plates which are shown on the drawing, shall be provided by Engineering on an appropriate design change. Construction shall transmit the removed name plate (s) to QC by a three-part memo. QC will transmit the name plate (s) to the Owner.
For B&R fabricated supports / parts that are to be salvaged and which contain welds, PFG is responsible for providing original documentation (WDC, WFML, etc. .) necessary for QC t'o accept the welds. This documentation shall become part of the new hanger !
package.
3.2.3.2 Re-Inspection of Salvaged Suppqrts ,
l When B&R or vendor supplied supports are designated for salvage ' l and returned to the warehouse, these supports shall be re-inspected by Receiving QC in accordance with Paragraph 3.2.3.3.
When B&R or vendor-supplied supports are to be salvaged, but not i returned to the warehouse, supports shall be re-inspected by ASME Field QCI in accordance with paragraph 3.2.3.4.
3.2.3.3 Salvaged Material Released from Warehouse ]
I Upon receipt of an MR from the warehouse for salvaged supports /
parts, the Receiving Inspector shall verify the following: )
- a. The MR identifies the intended use of the material or parts. j 1
- b. The MR identifies the original mark or supposrt number from l which it was salvaged--if the part being salvaged is a Code I stamped part;
- c. All material, parts, or assemblies are identified by a unique mark or assembly number, traceable to the original support identification for vendor-supplied Code stamped parts; .
- d. B&R fabricated component support parts or materials exhibit a heat number which is traceable to an available material certification;
- e. All vendor-supplied support parts, materials, or assemblies are traceable to an available material certification.
Upon completion of the above verification, the Receiving Inspec-tor will initial and date the MR.

1 3.2.3.4 Inspection After Release to Field l
Once salvaged parts, materials and/or assemblies are issued to -
the field for the construction, QC shall perform the following verifications: i
- a. All vendor supplied support parts or assemblies exhibit the unique identification number as shown on the corresponding MR or vendor ' data package; and
- b. All B&R fabricated parts and material exhibit the heat {
number as shown on the MR, if released from the warehouse.

When Category II supports attach to class 1, 2 or 3 structures, the connecting veld (s) shall be shown on the class 1. 2 or 3 ,

However, for welds on component standard supports (catalog items) which are intended to be " full length welds" by design, the weld length shall have a -1/16" tolerance (provided the actual length is equal to that of the installed component.)
Where the veld size noted on the design drawing is equal to the ,

- a. A minimum of 2" on each side of the weld joint shall be degreased prior to welding. Also, a minimum of 12".on each side of the weld join ~t shall be free of moisture, dirt and oil. The cleaning agent shall be either Vorsol 1 or Stoddard Solvent or beher approved solvents.
In addition the critria delineated in Para. 3.4.1.1 shall also apply.
3.4.5.2 Fit-Up Inspection -
Fit-up inspection, when required by process control documentation, shall be as follows:

3.4.5.3 Acceptance' Criteria As welded surfaces are permitged, however, the surface of welds should be free from coarse ripples, grooves, overlaps, and abrupt ridges and valleys to meet the following:
- a. The surface condition of the finished weld shall be suitable for the proper interpretation of required nondestructive examinations of the welds.
- b. Undercut shall not exceed 1/32" or r. educe the base metal below the minimum wall thickness.
- c. The weld joint and adjacent base metal shall also be free of defects such as, cracks, incomplete penetration, incomplete fusion, slag, blow holes, porosity, . burn through, are strikes, veld spatter, and mishandling marks.
In addition, inspection criteria (when applicable) in Para. 3.4.4 shall also apply.
3.4.5.4 Installation of Permanent Attachments to Piping System After Testing Permanent attachments may be welded to the piping system after performance of the pressure test provided that:

3.7.3.1 Bolt Modification Modification of bolts requires prior approval of the Design !
Engineer. When modification is authorized, QC shall witness the onsite. shortening, re-threading and stamping.
Bolts that are to be shortened and/or re-threaded shall be I checked prior to cutting by the QC Inspector to verify original' j length code and acceptability of thread fit. The site fabricated threads ' shall have the same fit as the factory threads. Fit shall be verified by comparing tightness / looseness of site fabricated threads to vendor threads by turning on a nut. After the bolt has been shortened, the QC Inspector shall verify the bolt has been correctly identified as to its length per Attach-ment 14. ,
'3.7.4 Embedment Length Concrete anchor bolts shall be installed into structural concrete with the minimum embedment lengths as shown on the design draw-ings. Where not shown on the design drawings, the minimum embedment length shall be as shown on Attachment 15.
Bolt embedment shall be determined by subtracting the length of the bolt extending from the structural concrete surface, from overall bolt length.

If no letter designation exists on the bolt, the OCI shall l request that Ultrascric Testing (UT) be perfer ed in accordance with QI-QAP-10.2-4B to determine bolt length; no UT report is required. 3.7.5 Bolt Installation / Setting of Wedges Bolts shall not be installed in concrete having strength less than the 28-day-old design strength. When w
,itnessing bolt installation, the QC Inspector shall verify ' bolt threads are protected by use of. double nuts or mandrel prio: to driving the bolt, and bolts do not visually indicate any cracks. Bolts that are bent, cracked, or have thread damage shall not be used.
Hilti bolts shall not be skewed , by more than 6*. Beveled washer (s) shall be installed when the angle between the hilti nut and it's mating surf ace is from 2*-6* inclusive, without drawing depicting this arrangement.
Bolts shall be set by torquing the nut. As a minimum, QCI shall' verify the torquing of at least one (1) cencrete anchor bolt per base plate. Anchor bolts shall be torqued to the values shown in Attachment 14.
The engagement of the nut shall be such that after torquing, the end of the bolt is not lower than flush with the top of the nut.
When initial torquing (or setting) of a bolt the change in bolt projection shall not exceed one (1) nut thickness, unless other-vise approved by the engineer.
When torque is verified, the nut may turn additionally due to the initial relaxation. Torque must be obtained prior to the nut bottoming-oyt in the threads. Minimum length of threads for the various sizes of Hilti bolts may be found in Attachment 15. If necessary, thread engagement may be verified by removing the nut.
If the torque value cannot be achieved, or if the pullout is greater than one (1) nut thickness, the bolt sh:11 be documented as "Unsat" on Attachment 13 and all bolts on that baseplate shall be torque tested.
Setting the wedges on Hilti bolts shall be verified by QC prior to grouting.

' measuring distance .between bolts, it shall be measured from center to center,of the bolts.
Spacing violations shall be documented in accordance with Para.
2.4. When these dimensions are shown on the drawing, they shall be considered a minimum. ,
3.7.7 Anchor Bolt Holes Reworked or Relocated Where embedment requirements cannot be met or bolts slip, loosen, pull out or cannot be set, the existing bolt location may be reworked or abandoned and relocated as follows:
3.7.7.1 Reworked Anchor Bolt Holes When the existing bolt hole is to be reworked. Quality Control shall be notified prior to commencing work and the following shall be verified: -
- a. The bolt is removed carefully without damage to the struc-tural concrete and without excessive spalling at the sur-face, per Paragraph 3.7.8 of this instruction;
- b. Wedges are removed from hole; and
- c. The bolt hole is re-drilled so that the new embedment depth is a minimum-of 4h anchor bolt diameters for Hilti Kwik Bolt or 6 anchor bolt diameters for Hilti Super Kwik bolts greater than the previous existing embedment depth.
3.7.7.2 Replacement Anchor Bolt When a re-drilled hole cannot meet the embedment depth as speci- )
fied in Paragraph 3.7.7.1.c. a new expansion anchor of at least ,

3.8.2 Inspection Criteria The QCI shall verify that the restraint conforms to the design drawing and the following:
- a. Stainless steel cable is of the proper size.
- b. Stainless steel cable is not frayed or broken.
- c. Stainless steel cable end loops are bent smooth without any deformation.
- d. Sufficient slack exists in the cable.
This can be verified if:
- 1. A visual gap 1/8" minimum exists between the inside of the eye-nut and the cable (as shown) is in the relaxed condition.
- 2. If the cable bears against the eye-nut, then a 1/8" diameter rod should slide between the cable and the eye nut using normal hand pressure.
NOTE: If 'there are two cables bundled together, the amount of slack shall be approximately equal.
This inspection shall be made after the eye-nut has been torqued to the Hilti Bolt; or tensioned snug tight when using other anchors. Crosby clamp torquing shall also precede this inspection (as applicable.)

For visual inspection, only indications with major dimensions greater than 1/16" shall be considered relevant indications; cracks are not acceptable. ! The weld joint and adjacent base metal shall also be free of defects such as incomplete fusion incomplete penetration slag and arc strikes. ATTACHMENT 11 l 1 l l SKEWED FILLET WELD MEASUREdENT 0? STANCHIONS The following steps shall be followed to verify stanchion weld sizes.
- 1. The actual area of weld to be measured is defined on continuation sheet 1 (Ref. Type "L" and Type "S").
~
- 2. Starting at 90*, mark off pipe as shoyn in Figure 1, at approximately 2" on center, for stanchion larger than 8" in diameter, mark off 3" on center. Number marks clockwise on the pipe and worksheet and show the orientation of weld on the worksheet (i.e., eleyation view looking west.) A mark should be placed at any point where the veld is visually smaller than the rest of the veld and measured.
- 3. Press the contour gage against tae veld at the points marked above. !
, Place the contour gage as perpendicular to the . face of the veld as ; possible. '
- 4. Prepare a worksheet per Fig. 6. The hanger and NCR number, if applicable, must be on the worksheet.
- 5. Transfer the weld configuration to the worksheet (see Fig. 2 and Fig.
- 6) and number each transfer with the corresponding number given in Fig. 1.
- 6. Use a straight edge and establish tangent lines as shown at Fig. 3.
- 7. Using a protractor, determine the angle of the obtuse fillet. Fillet welds with an angle greater than 135' do not require inspection for size. Note on worksheet that the welds greater than 135* have n.o t been measured. (Fig. 5 and Fig. 6 have examples of how fillet size is determined on the worksheet.

- 8. Draw a parallel dotted line as shown in Figure 5 at a perpendicular distance equal to the drawing required weld leg size. This dotted line must pass thru the toe of the weld to be acceptable.
- 9. Completed worksheets must be signed and dated by the QCI and put in the applicable hanger package.

T.his appendix describes the controls to be used for the modification, repair or replacement of Unit I and Common ASME Class 1,2,3 and Siesmic Category' II pipe supports < All modification, repair or replacement a'ctivities shall be performed per the in-spection criteria, requirements and methods delineated in QI-QAP-11.1-28. QCI shall inspect and complete the applicable in process documentation for all changes on the component support when any physical work was performed. Additionally, QC shall com-plete the applicable Inspection Reports in QI-QAP-11.-1-28 for modification portions of these supports. During QC in-process, attributes which are identified as unacceptable shall be documen,ted in accordance with CP-QP-16.0. Additionally, QCI shall identify these unsat attributes on the Process Control Document and the applicable QC Inspection Report. The deficiencies shall be corrected by establishing the necessary actions on the process control document and re-establishing the required QC/Afill in-spection hold point (s). Deficiencies discovered which are out of the scope of the in-process work shall be documented in accordance with CP-QP-16.0. Drawing re. visions that do not require any physical work to be performed on the pipe support shall be verified and documented'on the Hultiple Weld Data Card (MWDC)only. Inspection reports from QI-QAP-11.1-28 are not required when no physical rework is performed. All unsat attributes shall be documented in accordance with cP-QP-16.0. The following delineates the exceptions and actions required for implementing the transition for existing packages to the new Section XI Program.
