Sc::: nary (HIS) form ('s) ntst be conpleted for each supprt with attachnent(s) which affect the qualification of etedded plates in the thit 2 Safeguards, Containment, Ciesel Generator alildings, and Pipe Tunnel Area. Buildig attachnents which affect enbed qualification are attachnents welded directly to the enbedded plates, or attachnents via concrete expansion anchors /Ric'rmord Inserts that are closer to a loaded enbedded plate than the :ninimtm distance required.
