1.0 GENERAL 1.1 PURPOSE i 1.1.1 This procedure establishes location and spacing criteria for I all attachments on embedded plates in the walls, beams and ceilings of plant buildings to assure conformance with re-quirements of Specification 2323-SS-30. PROCEDURE EFFECTIVE CPSES NUMBER REVISION DATE PAGE-(s/ } JOB 351195 CCP-45 4 9/30/86 2 of 7 1.2 SCOPE 1 1.2.1 This document establishes location and spacing criteria for j all permanent (BOP and Non-BOP) and temporary attachments on embedded plates. In addition, this procedure controls the transmittal of infor-mation pertaining to BOP attachments on embedded plates and cites the reference documents which establish these criteria. The requirements of this procedure shall be followed except when specifically detailed otherwise on the originating en-gineering organization's drawing or upon issue of a pertinent design change. 1.3 GENERAL DISCUSSION 1.3.1 This document addresses the 3/4" thick plates anchored to the building walls, beams, and ceilings by Nelson Studs, attach-ments to these plates, and the location of these attachments on the plates. These plates provide structurally competent surfaces to which attachments such as pipe supports, cable tray supports, 3 -s HVAC supports, etc., can be permanently attached by welding. The j need for proper location of the supports on the plates is to avoid placing loads in areas of insufficient strength due to Nelson Stud location or proximity to the edge of plate. This procedure does not ' apply to embedded plates which are intended for permanent attachment of specific structures (e.g. platforms, stairways, equipment, e tc.). 1.4 RES PONSIBILITY 1.4.1 The proper location, minimum spacing requirements of attach-ments, and the compliance with this procedure shall be the responsibility of the superintendent of the craf t engaged in the installation of the attachments, i l 1.5 DEFINITIONS 1 1.5.1 " Strip Plates" are steel plates 3/4" thick, either 8" or 10" wide and of various lengths which are anchored to concrete surfaces by embedded Nelson Studs. 1.5.2 "Large Pates" are steel plates 3/4" X 4'-0" X 8'-0" or 3/4 X 9'-0" X12'-0" which are anchored to concrete surfaces by em-bedded Nelson Studs. " FOOTPRINT LOAD TRANSMITTALS" (FFL) are controlled documents which contain attachment size and location information and forces / moments applied at the face of an embedded plate. See. 1.5.4 " LOADED NELSON STUDS" are those studs located within 12" along the length of a strip plate or within 12" along the length or width of a large plate, measured from the face of an attach-ment. See Attachments 8 and 9. 1.5.5 " STRUCTURAL DiBEDMENT GROUP" (SEG) previously known as WEB is the engineering organization responsible for evaluating and qualifying embedded plates. l 2.0 PERMANENT ATTACHMENTS i l 2.1 LOC ATION 2.1.1 New or rework attachments to embedded plates in Unit I require Civil /SEC approval (indicated by the engineer's signature on an FPL, see Attachment 3) prior to installation. In Unit II, ,s ( ) only those installations that do not adhere to the design document N' / or this procedure require prior approval. 2.1.2 Atachments to large and strip embedded plates shall be separated a minimum of 12" from the face of an adjacent attachment (s) (See, Figure 2), unless specifically shown on the design document. The 12" minimum shall be maintained even if the adja-cent supports are located on different plates. (See Attachment 10). When the 12" separation cannot be maintained due to field conditions, the responsible engineer shall be notified prior to in s talla tion. An evaluation of this condition shall be performed and if found acceptable, this condition shall be shown on a design doc ument. Separation criteria do not apply to 1/4" diameter Nelson studs welded to the embedded plate for the purpose of supporting con-duit identfication tags. Unless specifically approved otherwise, welding shall be done using a maximum interpass temperature of 350* F on 3/4" embedded plates. ] 2.1.3 Loaded studs due to attachments te embedded plates shall provide the required clearance indicated in Attachments 5, 8, and 9, unless otherwise shown on a design document. If the stud location is not known, the minimum separation to an opening or loaded Hilti bolt / I Richmond insert shall be measured as described in Attachment 8 of this procedure. 2.1.4 Embedded plates which have separation violations with concrete 7s ( expansion anchors or Richmond inserts may be rendered unusable N/ by the Civil /SEC Group af ter an engineering evaluation has been made. This area of the plate will then be painted red by Field Engineering in accordance with the ap-propriate architectural specification. No attachment's shall be made to the portion of a plate which has been rendered un-usable. No portion of a plate shall be rendered unusable if it has an attachment already made to it. 2.2 "LARGE PLATES" 2.2.1 Attachments to "large plates" in the cantilever region as defined on Figure 1 of Attachment I shall not be made except in those cases where the design document specifically shows the attachment in the cantilever region. (See Attachment 7, Cases 1 through 3 for acceptability.) 2.3 " STRIP PLATES"
- 2. 3.1 The centerline of an attachment shall be within 3/4" of the centerline of a strip plate unless specifically shown on a design document. When the above tolerance cannot be main-tained due to field conditions, the responsible engineer shall
( be notified prior to installation. An evaluation of this ccr.- dition shall be performed and if found acceptable, this condi-tion shall be shown on a design document. ' 2.3.2 Attachments to " strip plates" in the cantilever region as defined on Attachment I shall not be made except in those cases where the design document specifically shows the attach-ment in the cantilever region. (See Attachment 7, Cases 1 through 3 for acceptability.) 2.4 " ATTACHMENT TO EMBEDDED PLATE" TRAVELER 2.4.1 New or rework attachments shall have an " Attachment to Em-bedded Plate" traveler issued by the Paper Flow Group (PFG). For attachments in Unit I and common, an approved FPL must be attached to the traveler before support installation. If an approved FPL is not attached to the traveler, the responsible engineer shall be notified and an FPL originated. Attachments to Unit II may be installed without an approved FPL provided the support can be located according to design documents or the criteria established in this procedure. Unless specifically approved otherwise, welding shall be done in accordance with W2S-10046, with maxi-mum interpass temperature as stated in paragraph 2.1.2. At-tachment shall be made by fillet welds, the size of which shall not exceed b inch. 3.1.4 Removal of attachments may be accomplished either by are gouging, or by gas flame cutting or by sawing. In either case, the attach-ment shall not be cut closer than 1/4 inch to the embedded plate. The embedded plate shall be ground smooth af ter this cutting. l' 3.1.5 Temporary attachments to embeddod plates shall be only the types shown on Attachment 2 of this procedure. 3.1.6 Attachments to ceiling plates, plates on the bottom of beams, and brackets to embedded wall plates shall not individually carry l suspended loads greater than those values shown on Attachment 2. 3.2 COMPLETION OF "TDiPORARY ATTACHMENT TO EMBEDDED PLATES" TRAVELER 3.2.1 The responsible craf t shall request the PFG to generate an operation traveler (Attachment 4) prior to the installation of temporary attachments to embedded plates. 3.2.2 Traveler shall be controlled and processed in accordance with Reference 4.8. 3.2.3 STEP I The responsible craf t shall enter the member type, the ap-proximate load, and the distance from the load point to the face of the embedded plate in the appropriate spaces and ini-tial and date in the column labeled construction. 3.2.5 STEP II In Unit I and common Civil /SEC approval is required before in-stalling a temporary attachment. This approval is required in Unit II oniv if the support does not conform to the guidelines of Section 2. If the support does conform to the guidelines of Section 2 QC shall N/A Step II. O) (_

If unsat condition occurs relocation of the support is required. 3.2.7 STEP IV Af ter support removal the responsible craf t shall initial and date in the appropriate column. The QC inspector shall inspect the removal area. If the removal area is unsatisfactory, the Inspection Report No. shall be entered by QC. The Repair Process Sheet No. shall be entered by PFC.

CCP-45 Revision 4 "CCP-45, Revision 4 was issued 9/30/86 as a major rewrite with signi-ficant changes in the documentation requirements for attachments to em-bedded weld plates. This revision was distributed based on the presup-position that logic would dictate when and how the documentation transi-tion would occur based on this and other related procedures. However, it has been brought to my attention a certain amount of confusion may exist with regards to proper documentation proceedings. The following elucidation should be distributed to all control copy holders of CCP-45, Revision 4. It must be rationalized that fabrication and documentation for attachments to embedded plates may have been at any of the belov stages when CCP-45, Revision 4 was issued and should be treated accordingly: 1. Attachments not installed - installation and documentation in accordance with CCP-45, Revision 4. O-l 2. Attachment installation completed - installation and docu-mentation in accordance with CCP-45, Revision 3 is accept-able. 3. Attachment installation and/or removal inprocess - a com-bination of documentation in accordance with CCP-45, Revi-sion 3 and Revision 4, as applicable, commensurate with the stage of activity for a given attachment. For these amal-gama ted packages, it will require that operations previously accomplished per CCP-45, Rev. 3 be marked "NA" on the super-ceding document issued per CCP-45, Rev. 4. I i INTERIM DISPOSITION This interim disposition does not address the inspection of previously ins'alled attachments in Unit II. Another interim disposition'will be t prepared by December 19, 1986 to address Unit II. 1) ROOT CAUSE a) (Reference items 1 and 4). The development of Quality Control procedures regarding the inspection of embedded plates did not conform to the requirements of CP-QP-6.0. b) (Reference items 2 and 3). The engineering review of Construction and Quality Control procedures pertaining to embedded plates was inadequate. i 2) ACTIONS PLANNED TO CORRECT PROBLEM a) Revise Specification SS-30 and Civil Engineeri'ng Instruction CEI-20 to clarify an illustration depicting concrete anchor violations. SUMMARY
To ensure the structural integrity of all loaded embedded plates in Unit I, a complete backfit inspection of the embedded plates will be performed. Field Engineering will generate as-built sketches. Civil /SEG Engineering will review the as-built sketches and Q.C. will field verify the information.' Conditions outside of Quality Control procedure QI-QP-11.14'-13 will be documented via NCR. Civil'/SEG will disposition the NCR's. PURPOSE These instructions contain the criteria.to be used by Field Engineering to generate embedded plate as-built sketches and tolerances for Q.C. verification. l-SCOPE 4 l As-built sketches depicting attachment locations and spacing dimensions shall be generated for every " Loaded Embedded Plate" in Unit I only. All " Loaded Embedded Plates" shall be accounted for on a defined boundary basis. ' DEFINITIONS 1 F.E. Sketch - Field Engineering sketches are field generated sketches that depict the location of embedded plates and information regarding attachment locations and spacing dimensions affecting embedded plate attachment analysis. l Inspection Package - Inspection packages define accountability traveler and all individual travelers within a room / area. Loaded Embedded Plate - Loaded embeds are steel plates flush with the concrete surface and anchored to the concrete with Nelson studs which through existing welded attachments transfer loads to the concrete structure. e j i .,.s. - - i u..- - --.+.'

"Ex" - is the attachment location eccentricity from the embedded plate center line. s "F" - is the distance from the edge of an embedded plate to the edge of a penetration or edge'of concrete. "D" - is the distance from a Richmond insert to a loaded Nelson stud (centerline to centerline). "Z"- .is the distance fro's a Hilti bolt to a loaded Nelson stud (centerline to contariin'e). "R" - is'the radius (minim:tm required distance) from loaded Nelson stud to Hilti' bolt, Richmond insert, or free edge of concrete. (Ref. Attachment 7, 8, and 9). I "S" - is the distance from the face of a welded attachment to the 1 face of an adjacent welded attachment. 1 "O" - is the distance form the face of a welded attachment to the edge of a hole on the plate. For illustration purposes see attachment 1. 1 CONSTRUCTION TRAVELER CONTROL The Paper Flow Group will issue construction travelers as requested by Field Engineering. Field Engineering will attach an as-built sketch to . every embedded plate traveler and send the travelers to the Structural Embedment Group for review and approval of as-built sketch. SEG will then release inspection packagee to Quality Control for backfit inspection. Upon completion of the inspection, QO will forward the 1 inspection package to PFG for retention.
