reviews and approvals of the Authorized Nuclear Inservice Inspector (ANII) are made as required by the ASME Boiler and Pressure Vessel Code.

2.0 Applicability .

This instruction is applicable to all interface activities of the Maintenance Department with the ANII, to assure the ANII has the opportunity to make the necessary inspections, verifications and

- reviews as specified in Paragraph 4.1.3 for Units 1 and 2, as the ANII deems necessary to permit certifying that, to the best of the ANII's knowledge and belief, all examinations, tests and repairs or l

replacements comply with the requirements of the ASME Boiler and Pressure Vessel Code Section XI. This instruction becomes effective ,

l when issued.

3.0 Definitions L'l 3.1 Examination - Denotes the performance of all visual observation and nondestructive testing, such as radiography, ultrasonic, addy current, liquid penetrant and magnetic particle methods.

3.2 Inspection - Denotes verifying the performance of examinations and tests by an inspector representing an Authorized Inspection Agency.

3.3 ANII - Denotes an Authorized Nuclear Inservice Inspector as defined and qualified by the requirements of ANSI /ASME N626.1-1975.

3.4 Inspector - Denotes an Authorized Nuclear Inservice Inspector as l defined in ASME Section XI, IWA-2130.

3.5 Test - Denotes the determination of operational readiness by such means as pressure or inservice tests.

i 3.6 Section XI - Denotes the use of requirements as deca 11ed in ASME Boiler and Pressure Vessel Code Section XI, as required by the current revision of 10CFR50.55a, Codes and Standards. ,

f 3.7 Maintenance Action Recuest (MA'.) - A document used to initiate, control and document maintenance work which must be performed on station equipment.

( AUTHORIZED NUCLEAR INSERVICE INSPECTOR INTERFACES REVISION NO. 1 PAGE 3 0F 10 3.8 Weld Record - A form used to identify specific welding and inspection requirements and to record: Filler Metal used; Weld Identification; Welder Identification; Inspection and Examination Results and Authorization and Approval Signatures.

3.9 ISI Ten Year Plan - A program which defines the specific Inservice Inspection (ISI) requirements for the ASME Section III Class 1, 2 and 3 systems and components during the first inspection interval (10 years following initial start of power unit commercial operation).

4.0 Instructions 4.1 Responsibilities 4.1.1 The Maintenance Engineer shall be responsible for designating a Coordinator to act as the point of contact between the plant staff and the ANII for all ASME Sections XI Code activities. In the event the Coordinator is not available, the Maintenance Engineer or NDE Level III should be the point of contact with the ANII in his absence.

4.1.2 The Coordinator shall be responsible for the following:

4.1.2.1 Ensure that the applicable procedures and records are available for the ANII to fulfill his responsibilities stated in Paragraph 4.1.3.

4.1.2.2 Notify the ANII of the following listed Section XI activities prior to initiation of work:

4.1.2.2.1 Welding - pressure retaining components and or parts and component supports; 4.1.2.2.2 Hydrostatic tests associated with repairs; 4.1.2.2.3 NDE associated with welding and/or evaluation of indications; 4.1.2.2.4 NDE and Visual Tests associated with' the Inservice Inspection (ISI) Ten Year Plan; -

examination (s) and test (s) required by the ASME Section XI Code.

4.1.2.2.5 Metal removai associated with repair (s) or evaluation of NDE indications.

4.1.2.3 Inform the ANII of the status of ASME Section XI activities outside the scope of activities defined 'in 4.1.2.2 above.

4.1.3 The ANII shall be responsible for the following:

. 4.1.3.1 Verify the required examinations and system hydrostatic test.

4.1.3.2 Ensure that the required visual examinations b_/ VT-1, VT-2, VT-3 and VT-4 as required by Section XI have been performed and the results recorded.

4.1.3.3 verify that the system pressure tests, other than

  • system hydrostatic tests, where required for Class 1, Class 2, Class 3 and Class MC components have been conducted and the results recorded.

4.1.3.4 Perform any additional investigations necessary to verify that all applicable requirements of the above have been met.

4.1.3.5 Verify that the inservice tests required on pumps, valves and component supports as required by Section XI have been completed and the results recorded.

4.1.3.6 Verify th,at the nondestructive examination methods used follow the techniques specified in Section II.

4.1.3.6.1 Verify that the examinations are performed in accordance with written qualified procedures and by personnel employed by the Owner or the Owner's agent and qualified in accordance with Section XI.

for the technical content and requirements of the examination procedures and the qualification procedures of nondestructive examination personnel.

4.1.3.7 Ensure the requalification of any examination procedure or operator if the Inspector has reason to believe the requirements are not being met.

4.1.3.8 Certify the examination records only after verifying that the requirements have been met and that the records are correct.

4.1.3.9 Verify that repairs are performed in accordance with the requirements of the Owner's repair s

program.

4.1.3.10 Review the Quality Assurance Program for repairs and ensure its implementation.

bf;7 4.2 Notification. Informing and Authorization 4.2.1 The ANII shall be notified prior to commencing any ASME Section XI activities identified in 4.1.2.2. .

4.2.1.1 Notification shall indicate to the ANII that the MAR and associated records such as the Weld Data Record Form (Attachment 1), (refer to WLD-104) or Maintenance Contractor Process Control records are available for review and assignment of witness points, as applicable.

NOTE: All ASME Section XI activities shall be initiated by a Maintenance Action Request (MAR) in accordance with MEI-004, " MAR Review Processing."

4.2.2 The ANII shall be informed of the status of ASME Section XI activities outside the scope of the activities identified in 4.1.2.2, which the inspector may elect to witness or verify.

4.2.2.1 The ANII shall be kept informed of Section XI activity statuses by the following methods:

In addition, tracking codes are provided which indicate the physical location of each MAR.

4.2.2.1.2 The MAR Coordinator will be the Inspectors contact for utilization of the tracking codes provided in the MAR Tracking Report.

4.2.2.1.3 Terminal access for Maintenance activities will be available to the Inspector upon request.

t 4.2.3 All ANII Notifications shall be documented as follows:  !

4.2.3.2 All off site Notifications shall be documented on  !

the Authorized Nuclear Inspector Notification and Authorization Log (Attachment 2).  !

4.2.3.2.1 All MARS and WDRs applicable to the activities shall be recorded in the log.

4.2.3.2.2 Notifications involving Maintenance Contractors shall be noted in the j comments section of the log.

i 4.2.3.2.3 The ANII's concurrence to commence work shall be documented by his signature and/or by telecon in the appropriate blocks within the ANII's Authorization to Commence Work section of the Log. I 4.2.3.2.4 When approval to commence work is given by telecon, the person who contacted the ANII shall indicate witness points imposed by the ANII on the MAR and/or Repair Plan as applicable.

4.2.3.2.5 If the ANII elects not to review the MAR, Repair Plan or assign witness points prior to commencing work activities, the ANII's " Authorization j

to Commence Work" signature block shall 'l be marked "not required" (NR).

4.2.3.2.6 The " Notified By" block under the ANII

" Notification Section" shall always be signed by the person who contacted the ANII.

4.2.4 Prior to commencing work, the Coordinator shall be notified by the MAR coordinator to assure all witness points designated by telecon are specified in the MAR and/or Repair Plan as applicable and when no witness points are required by the ANII, any predesignated ANII inspection points are marked NR, initialed and dated. e 4.2.5 A Telephone Conversation Record should be used in accordance with MEI-021, " Telephone Conversation i Documentation", to document the conversation with the

' k;j ANII. The Telephone Conversation Record should note the 1

following items (as applicable):

a. Date of contact.
b. Name of ANII contacted.
c. Probable start date of work.
d. Description of welding to be performed,
e. Examination and tests to be witnessed by the ANII.

4.2.6 The ANII shall be contacted at the following locations:

4.3.2 The above referenced logs are the official ANII records of notification and shall be maintained by the Maintenance Services Engineer. -

4.3.3 Policy changes which effect the ANII and owner interface which are documented by telephone conversation records, meeting notes, letter correspondence or interoffice memos, etc., will also be considered official ANII records and will be filed in the Maintenance Engineering miscellaneous files.
