1.0 REFERENCES
2.0 GENERAL 2.1 PURPOSE & SCOPE: These instructions establish the basic criteria for creating a BRHL (Brown & Root Hanger Location Isometric Drawing). 3.0 INSTRUCTION 3.1 The draftsman will receive a marked up BRP blue line print . from the BRHL group leader showing the total number of hangers, the approximate location of each hanger and each hanger number. 3.1.1 The draftsman will simultaneously receive from the BRiil group leader a sepia of the Brown & Root piping isometric (BRP) from which the marked-up blue print was made. 3.1.2 The draftsman will alter the prefix of the drawing number of i the sepia from "BRP" to "BRML". This will be done both in the drawing number block in the lower right of the sepia and where
; the drawing number appears in the upper left hand corner of, the sepia, between the drawing borr'er and the edge of the paper.

. 3.1.3 The draftsman will transfer all infonnation indicated on the ! marked-up blue line to the sepia. The hanger number will be * , given inside a rectangular block 1/4" x 2". A leader line will be drawn from the rectangular block to the approximate'
.j location of the hanger on the pipe and an "X" will be drawn on ,a the pipe at the contact point of the leader line and the pipe. 3.1.7 The draftsman will initial the BRHL sepia with his initials indicating he has drawn the BRHL. l 3.1.8 The draftsman will run a blue line check print of the BRHL and forward the BRHL sepia, the original marked-up blue line BRP and the BRHL blue line check print to the final checker. 3.1.9 The checker will assure all changes have been made correctly by comparing the marked-up blue line BRP print against the blue line BRHL check print. If the BRHL is not correct, it is returned to the draftsman for correction per 3.1. If the BRHL is without error, the final checker will initial the
- drawing as checked and forward all check prints and the BRHL sepia to the lead draftsman.
3.1.10 The lead draftsman will give the BRHL drawing a cursory check and initial the drawing as approved. 3.1.11 The 1ead draftsman will fonvard the entire drawing package to the clerk. , 3.1.12 The clerk will submit the BRHL to DCC for distribution and create a folder marked with the BRHL drawing number and con-3- lj taining all check prints. She will file this folder for re-ference. l

A Brown & Root (B&R) Millwright Superintendent stated I that hamers were used on the condenser tubes, but only when using a special tool to protect the tube ends. He also stated that this is a 9 l standard practice in condenser assembly. The Millwright f Superintendent disclaimed any knowledge of misaligned tube sheet
! holes.
,3 l The NRC also interviewed a TUEC Operations Results Engineering l l Supervisor concerning the availability of documents such as inspection reports, deficiency reports, in-progress monitoring reports, NDE 'I
! reports, and procedures related to the allegation. He stated that work was performed to B&R procedures and he did not keep 6ny official records, since his particular involvement was limited to

In addition, the Operations Results Engireering Supervisor stated that, during the rolling of the tubes, the optimum tube-wall reduction for a positive tube-to-sheet seal was in the range of 6 to 9 percent. He also stated that, at the beginning, when l ,,
experirr.enting with the torque on the rolling tool, the 9 percent reduction in wall thickness did not occur in some cases, but this did i not result in any of the tubes cracking. (For an explanation of tube cracking, see " Note" on the NRC staff's sketch, Exhibit A). t

In another interview, a B&R Millwright Foreman informed the NRC staff
, that he was not involved in the construction of the condenser and
! disclaimed any knowledge of the alleged problems. This person said it was rumored that the condensers, the auxiliary condensers, and the moisture separators might be retubed. This rumor was confirmed both
- 4 by the Millwright Superintendent and in a telecon with the TUEC Nuclear Ergineering Manager. The latter informed us that his group was conriissioned to make a feasibility study of condenser retubing, retubing Unit 2 first and retubing Unit I during the first outage.
The proposed change was to retube using titanium tubes instead of I

! On September 14, 1984, the NRC staff cortacted an Allis-Chalmers (A-C) representative who supplied information regarding part (d) of this allegation, installation progress reports show that the low-pressure .
I (LP) II condenser's expansion-joint weld was completed on September "! 18, 1978. The welding process was monitored during welding, and once in approximately every hour micrometer readings were taken at 12 1 points around the joint. These readings showed that the maximum
! movement was 0.026 inches (0.660 mm), which was acceptable to Allis
,i
- Chalmers, Westinghouse, and TUEC. Other than visual inspection, a
.i ~ l nondestructive examination (NDE) was not required. The same progress report shows that the LP-I expansion joint welding 1 l began on the same date. On October 13, 1978, Deviation Report No.14094 was written against the lower weld on this joint. The movement of 0.277 inches (7.030 mm) exceeded that allowed by A-C. Consequently, on October 19, 1978, the removal of the lower weld was . 4

j it is unlikely that any undue or undesirable stress was introduced 2 i ir:te the' welded joints. Following welding, the only NDEs performed 1 (other than visual inspections) were hydrostatic ar.d vacuum tests. The NRC staff also reviewed seven condenser hydrostatic test packages
- for the results of tests conducted in accordance with the G&H test
, procedure (Specification 2323-M-23). These tests spanned from December 1980 to April 1984. The NRC staff found that retests of the shell were made due to design changes. The last of these retests, for ' .{
I; example (No. ICO-0200E), was performed in September 1983 and involved l sodiui.1 tracer in3ection and sampling at points through the condenser wall. In all cases the tests were successfully completed. In May
.j 1984, TUEC performed a vacuum and water box priming retest (Procedure ll ICP-AT-27-01-RT-1) to again verify that the main condenser could be i

The NRC staff found no evidence that holes in the tube support sheets were misaligned by 3/8-inch, nor did they find evidence that the
; turbine to condenser was misaligned to the point that excessive stress I was introduced. The conclusive evidence is that the condenser was I
constructed following approved construction and testing procedures and
- as such, will perform its designed function. In addition, this unit is classified nonsafety-related and is not essential for the safe shutdown of the plant. Accordinglf, this allegation has neither i

Sept. 19, 1984 sui.;ui _ Installation Inspections of the Steam Generator Upper Lateral Supports. Ref: G&H 2323-Sl-550 This letter is written to clarify a possible point of confusion concerning the safety class and applicable code classification of the above referenced supports, and to provide a summary of the in-spection documentation produced to date attesting to their instal-lation. Gibbs & Hill drawing 2323-51-550, Rev. 4 provides the steam gen-erator upper lateral support details while drawing 2323-S1-544, Rev. 2 shows the support location and embedment details. The Westing-house drawing # 1457F34, for " Reactor Coolant System Equipment Sup-ports; Steam Generator Upper Lateral Support General Arrangement", shows the above mentioned G&H support as " support by others-typi-cal." Also, the Westinghouse analysis of the primary equipment supports, WCAP-10197, May 1984, Revision 2, Section 2-scope of analysis, clearly excludes the steam generator upper lateral cross compartment beams from their study. Further, NCR M906, (attach-ment 1) written 2-22-78, was dispositioned by issuance of two let-ters, GTM-25792 and TUS-5056 (attachment 2), determining the upper lateral support classification as Non-NF. With this distinction made the subject supports are considered integral to the contain-ment internal structure and are therefore not applicable as to safety class or code class as listed in table 17A-1 of CPSES/FSAR. They are however seismic category I and are constructed to the requirements of the AISC code. The supports were procured from AFCO Steel on purchase order # 35-1195-14915 with fabrication in accordance with Gibbs & Hill specification 2323-SS-17. Upon arrival at CPSES the following receiving inspection reports were written by Brown & Root Quality Control personnel: RIR# 05578, # 05579 and # 05838. Each RIR was supplemented by a " prior to off-loading checklist" and a " check-list for mincellaneous steel" which contained inspection points attributing to the condition of the item upon receipt, material
- marking requirements, conformance to purchasc order and Engineer-j ing specification, and the presence of all required documentation.
The following material received records were also signed by QC , personnel upon receipt of these items: MRR# 58072, # 58073 and
# 60752.
Warehouse material requisitions have been located which show the supports, embed plates, and shims were released to the building (rebar) department for installation in April of 1978, the 2h" diameter bolts necessary for installation were also released at this time to the Millwright department. r 3 <2ont. page 2 . This is consistent with the information gained through interviews conducted by myself with some of the craftsmen involved in the installation of these supports. According to the personal account of Jim Parnell, Building Dept. Foreman, the steam generator upper lateral supports, embed plates, shims and several bolts were assembled and lowered into position in each of the compartment.- to the location given on the drawings Sl-550 and SI-544. The assembly was tied in place by cadwelding the surrounding rebar to the "B" series cadweld sleeves on the embed plates. Once cadwelding was complete and all necessary shear ties and supplemental rebar was in place, the location was reverified by Field Engineering. QC Inspectors then verified the embedded steel placement was correct and authorized concrete placement to proceed. Lee Nabours, Mi.il-
, wright Journeyman, recalls being asked to assist in the installation of the remainder of the bolts for these supports. It seems there was some difficulty in installing the bolts due to misalignment of the holes in the shims with the threaded holes in the embed plates. The shim holes were enlarged by grinding away that por-tion of the shim that obstructed the threaded hole. The threaded holes were then cleaned by " chasing" the threads with a tap, com-pressed air was directed into the hole to remove loose debris, "neolube", was then applied to the bolt threads and the bolts :n-stalled to a snug tight condition. Mr. Nabours cannot recall the extent of OC involvement other than their presence.
Concrete pour cards # 101-4832-004 and # 101-7860-009 (attach-ment 3) have been located and each contain OC checklists which verify in part - embedded steel is in detailed location, is i rigidly secured, and safety related bolts and embe d ded metal mapped and documented to required procedure. The ..iscellaneous steel installation checklist in each pour card package further identifies embedded steel items by mark 4, purchase order # anc receiving inspection report 8. ASME Mechanical QC Inspectors originally used a ', relimanary installation and erection checklist" (attachment 4). The check- , list shown is incomplete with regard to installation criteria and does not specifically identify the supports as those required cn i 2323-S1-550. As stated earlier, the term " upper lateral supports" is also used for those items supplied by Westinghouse. The traveler shown (attachment 5) is written to cover the final adjustment of the steam generator upper lateral supports prior to hot functional test. It is not written to be an installation traveler although several of the operations included are pertinent installation activities. Note 3 of the traveler ! cludes 2323-I SI-550 as a reference drawing, operation # 3 loca- the centerline elevation of the bumper on the upper lateral beam, .nd operation
