REQUEST FOR TEMPORARY WAIVER OF COMPLIANCE -' TECHNICAL SPECIFICATION (TS) 4.4.5.2, TABLE 4.4-2, STEAM GENERATOR (SG) TUBE INSPECTION -

ARKANSAS NUCLEAR ONE, UNIT 2 (ANO-2)

This acknowledges the granting of a temporary waiver of compliance to AN0-2 from the provisions of TS 4.4.5.2, Table 4.4-2, " Steam Generator Tube m Inspection", to allow continued op9 ration of ANO-2 in light of missed inspections of two SG tubes. Your letter dated November 27, 1992, which describes the condition where two tubes in the "B" SG were not inspected with the bobbin coil probe during the ninth refueling outage (2R9, ending.0ctober .

1992), provides the written basis for the temporary waiver of complianc.e.

A temporary waiver of compliance was verbally granted on November 27, 1992, by

RC's Office of Nuclear Reactor Regulation (NRR), with the concurrence of NRC's Region IV Office. The waiver is effective for up-to 90 days, until a TS license amendment can be submitted and processed. The waiver was granted with the understanding that you will submit a TS license amendment request >'ithia 30 days regarding TS 4.4.5.2, Table 4.4-2, and the missed inspection of the two SG tubes. ,

, NRR performed an evaluation of your written. documentation and found that it justified the basis for your request for a . temporary waiver of compiiance. .As indicated in your letter, both tubes were inspected in 2R8 (March-1991) and there war no _ detectable degradation on-either tube. . Also, the 2R9 eddy current data for 28 tubes surrounding the two missed tubes (two rows of tubes around each of the two missed tubes) revealed only one indication-(28 percent :-

.Mr. Jerry W. Yelverton exist for circumferential cracking at the top of the tubesheet region of these tubes, since they were examined with the rotating pancake coil during 2R9.

With regards to compensatory mcasures, ANO-2 is currently operating at a reduced hot leg temperatura to mitigate the consequences of intergrannular stress corrosion cracking, a SG tube damage mechanism. SG tube leaks would be detected and confirmed by the main steam line N-16 monitors, condenser off-gas monitor, SG blowdown tonitors, main steara line radiation monitors, chemistry analysis, or reactor coolant system inventory balance. Also, ANO-2 has an administrative primary-to-secondary leakage limit of 0.1 gallon-per-minute (GPM), below the TS value of 0.5 GPM for leakage through any one SG.

Additionally, ANO-2 operator continuing training routinely stresses identification and mitigation of SG tube leaks by way of procedure review, classroom instruction and ongoing participation in simulator scenarios, and shif t briefings are being conducted to ensure cperations personnel are aware that two tubes in the "B" SG were not tested.

As indicated by your staff on November 27, 1992, we understand that the ANO Plant Safety Committee reviewed and approved the basis of your proposed request for a temporary waiver of compliance prior to requesting this waiver from NRC.
