INTERIM REPORT OF SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 96

" FAILURE OF CVCS PUMP " TRAIN A" TO START ON A SIAS" INTRODUCTION

. This report is submitted pursuant to 10CFR50.55(e). It describes a failure to CVCS pump train "A" to start on a SIAS. This problem is considered reportable under the requirements of 10CFR50.55(e).

To the best of our knowledge, this deficiency has not been reported to the USNRC pursuant to 10CFR21.'

DESCRIPTION During loss of offsite power testing, train "A" CVCS pump did not start on initiation of SIAS. Uponinvestigation,Oktwasfoundthatalatchingrelay malfunction (Electro Switch make) in the control circuit caused this problem]'

We have determined that the relay malfunction is because of excessive wear of

- components in the relay operating mecha' 'sm.

SAFETY IMPLICATIONS A single CVCS pump is taken credit for in certain small break LOCA analyses.

Failure of pump "A" to start as a result of the above explained relay malfunction would, therefore, violate single failure criteria, if left uncorrected.

CORRECTIVE ACTION

.Two relays which malfunctioned were sent to Electro Switch Company along with a request to evaluate all possible causes for the relay malfunction and provide a resolution in the form of permanent repair or redesign of the operating mechanism.

An Interim or Final Report will be submitted on or before March 2, 1984 I
