FINAL REPORT OF SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 84

" TUBE TRACK WELDING DEFICIENCIES" t

I INTRODUCTION This report is submitted pursuant to.10CFR50.55(e). It describes welding deficiencies on tube tracks used for the routing and support of

. instrumentation lines in safety related systems.

'To the best of our knowledge, this deficiency has not been reported to the USNRC pursuant to.10CFR21.

.pESCRIPTION During sample inspection of instrumentation tube track welding, it was discovered that the welding did not meet AWS D1.1 requirements. Also, the Lwelders who were qualified to weld in accordance with Mercury Welding Procedure WPS-B which requires a backing strap to initiate'the welding, were not using.the backing strap (i.e., open butt welding). At the present time, Ebasco Engineering has completed the evaluation of this deficiency and has authorized. corrective actions described below.

SAFETY IMPLICATIONS The tube track'in question supports the instrumentation. tubing for safety

related systems. The subject tubing leads;to instruments.that are required for plant parameter monitoring and safe shutdown of the plant. If the subject deficiencies would have resulted in a failure of the supports to perform their design functions, degradation could have occurred resulting in

-failure of the instruments to provide reliable information required by the reactor operators.

CORRECTIVE ACTION TAKEN Nonconformance Report W3-6159 was initiated to track and document this deficiency.-

FCR-1CP-561 was issued May.11, 1983 by Ebasco Engineering to provide guidelines.for implementation of corrective action. However, this course was

.not deemed practical.:therefore, Engineering conducted a test program to 4 determine the adequacy of'the welds as installed. The test program and-

, results are documented on Attachment #9 to the NCR, dated December 8, 1983.

i Based'on_ test results, it has-been determined that-the welds identified by the I .NCRias being deficient will perform their design function. The NCR is therefore closed with no rework required.

e-No1 corrective action on future tube track '.nstallations by Mercury Company is required-as Mercury is no longer performitg construction work'at Waterford 3.
